Zusammenfassung
Chirurgische Desobliteration und transluminäres Stenting der Karotis stehen im Wettbewerb um die Behandlung der Karotisläsion. Nach Erscheinen der großen randomisierten Multizenter-Studien ist die Indikation heute weitgehend geklärt. Die Zusammenfügung der Daten von NASCET und ECST hat zu weiterer Sicherheit bei der symptomatischen Karotisstenose geführt. Bezüglich der asymptomatischen Karotisstenose kann eine Indikation zum therapeutischen Vorgehen nur bei sehr niedriger periprozeduraler Komplikationsrate gesehen werden.
Vorteil operativen Vorgehens ist die Beseitigung des Plaque unter kontrollierten Bedingungen, Vorteil der Stentplatzierung die geringere Invasivität. Bei gleicher Komplikationsrate wird ohne Zweifel das Verfahren gewählt werden, das die geringere Invasivität aufweist. Die Entscheidung wird dann der Patient selbst treffen. Die Auswertung randomisierter Studien Operation versus Stenten (CREST, SPACE) werden Ende 2006 vorliegen.
Die Operation der Karotisstenose ist auch heute indiziert. Langfristig aber können nur niedrige Komplikationsraten, steigende Qualitätsanforderungen und intraoperative Qualitätskontrollen die Indikation bestätigen.
Die chirurgische Therapie der Karotisstenose wird zukünftig nur dann noch von Bedeutung sein, wenn es gelingt, die perioperative Komplikationsrate weiter deutlich abzusenken.
Summary
Surgical endarterectomy and transluminal stenting of the carotid artery have been the two alternatives in the treatment of carotid artery lesions. Publication of large multicenter trials has largely clarified current indications. Putting together the data from two trials, NASCET and ECST, has given greater certainty to the management of symptomatic carotid artery stenosis. With respect to asymptomatic carotid stenosis, indications for therapeutic procedures are given only if and where the periprocedural complication rate is very low.
Surgical intervention is of advantage in removing the plaque under controlled conditions, while stent placement has the advantage of lower invasiveness. If the complication rates are the same, the procedure will undoubtedly be chosen that is less invasive. The patient himself will then make the decision. Analysis of randomized trials of operation vs. stent placement (CREST, SPACE) will be published towards the end of 2006.
Carotid endarterectomy is still indicated nowadays. But in the long term the indications must be checked against low complication rate, increasing quality-of-life demands are met and intraoperative quality control.
Surgical treatment of carotid artery stenosis will be valuable only if significant lowering of the perioperative complication rate can be achieved.
