Palliative Chemotherapie

Fast 50 Jahre lang galt der Antimetabolit 5-Fluorouracil (FU) als die wichtigste Substanz in der palliativen zytostatischen Behandlung des kolorektalen Karzinoms. Die durch protrahierte Infusion über ein oder mehrere Tage und/oder biochemische Modulation mittels Leukovorin (LV) realisierbaren etwas höheren, dennoch nur bei rund 20 % gelegenen, therapeutischen Ansprechraten stellten die wichtigsten Erkenntnisse zahlreicher klinischer Studien der 80er und 90er Jahre dar. Erfreulicherweise haben sich die Behandlungsmöglichkeiten durch die Entwicklung neuer Substanzen, vornehmlich der oralen Fluoropyrimidine, der Topoisomerase I-interaktiven Substanz Irinotecan und des neuen Platinanalogons Oxaliplatin inzwischen wesentlich verbessert [9]. Monoklonale Antikörper gegen den epidemialen Wachstumsfaktorenrezeptor (EGFR) bzw. gegen den vaskulären endothelialen Wachstumsfaktor (VEGF) haben in Kombination mit Chemotherapie ebenfalls vielversprechende Ergebnisse gezeigt und werden das therapeutische Spektrum zweifellos noch mehr bereichern.
