Anwendung standardisierter Assessments für Kreuzschmerzpatienten: Einfluss auf das Clinical Reasoning von Physiotherapeuten[1]

Zusammenfassung
Hintergrund: Assessments für Patienten mit chronischen Kreuzschmerzen müssen Messungen beinhalten, mit denen das Aktivitäts- und Partizipationsniveau des Patienten, seine persönliche Sichtweise des Problems und seine Art der Schmerzverarbeitung erfasst werden können. In einem Physiotherapieteam eines mittelgroßen Krankenhauses wurden 2 standardisierte Assessments (Oswestry Disability Index [ODI] und Coping Strategies Questionnaire [CSQ]) implementiert, um diese Aspekte des Assessment zu untersuchen.
Ziel: Evaluation, welche Auswirkungen die Anwendung ausgewählter standardisierter Assessments für Patienten mit chronischen Kreuzschmerzen auf den Clinical-Reasoning-Prozess der Physiotherapeuten hat.
Methoden: Anhand von Fokusgruppen wurden die Erfahrungen der Physiotherapeuten mit der Anwendung der Fragebogen ausgewertet und die Daten wurden in Bedeutungs- und Themenkategorien eingeteilt.
Ergebnisse: Die Anwendung standardisierter Assessments hatte unmittelbare Auswirkungen auf das Clinical Reasoning und die Entscheidungsfindung der Physiotherapeuten. Auch das Verhältnis wischen Patienten und Therapeuten wurde durch die Assessments beeinflusst. Eine differenziertere Anwendung von Fragebogen wurde als notwendig erachtet. Die Eingliederung von Assessments in die Behandlungsstandards wurde als sinnvolle Praxisänderung befürwortet. Kontinuierliche Reflexion, Diskussionen und die erforderliche Erweiterung des Wissens über die Behandlung von chronischen Schmerzen wurden als notwendig erachtet. Es müssen Mittel und Wege etabliert werden, um die durch Fragebogen ermittelten Befunde interdisziplinär zu kommunizieren.
Schlussfolgerungen: Die Anwendung von standardisierten Assessments hatte positive Auswirkungen auf das Clinical Reasoning der Physiotherapeuten. Sie erfordert sorgfältiges Abwägen, Kommunikation und einen fortgeschrittenen Wissensstand.
Abstract
Background: Assessments for chronic low back pain patients must include measures targeting the activity and participation level, the patients’ personal view of their problem and their pain management. In a middle-sized hospital’s physiotherapy team 2 standardised assessments (Oswestry Disability Index [ODI] and Coping Strategies Questionnaire [CSQ]) were implemented to investigate these aspects of assessment.
Objective: Evaluation of the impact of selected standardised assessments for chronic low back pain patients on physiotherapists’ clinical reasoning processes.
Methods: Focus groups were used to assess the physiotherapists’ experience with questionnaire usage. Data were evaluated into a categorising framework of significance and emerging topics.
Results: The use of standardised assessments had immediate effects on physiotherapists’ clinical reasoning and decision making. The assessments also affected the patient-therapist relationship. A more differentiated application of the questionnaires was considered as necessary. Integration of assessments into treatment standards was supported as a reasonable change in practice. Continuous reflection, discussion and necessary knowledge expansion concerning chronic pain management was considered as necessary. For an interdisciplinary communication of questionnaire findings ways and means must be established.
Conclusion: The use of standardised assessments had a positive impact on physiotherapists’ clinical reasoning. The application necessitates careful balancing, communication and advanced knowledge.
