Intima-Media-Verdickung: Indikation für Acetylsalicylsäure-Behandlung?

Fall 1: Ein 56-jähriger, schlanker Mann ist bis auf eine mit Metoprolol 50 mg behandelte leichte Hypertonie gesund. Der Riva-Rocci-Wert ist unter Therapie meist 130 – 140/80 mm Hg. Cholesterin 156, HDL 51, LDL 90, Triglyceride 73. Doch zeigte die Carotisdoppleruntersuchung eine Intima-Media-Dicke (IMT) von 1,6 mm beidseits „in der gesamten Strombahn inklusive der proximalen Abschnitte der A. carotis interna und A. carotis externa, kein Plaquenachweis, Bulbusektasie von 13 mm”. Etwa 2 Jahre zuvor betrug der IMT-Wert noch 1 mm. Welche Bedeutung und Konsequenz hat ein IMT von 1,6 mm?

Fall 2: Ein zweiter, etwas älterer Mann mit exzellenten Risikowerten mit einer Intima-Media-Dicke von 1,2 mm. Der Kardiologe empfiehlt Acetylsalicylsäure (ASS) 100, doch hatte der Patient mehrfach Ulcera ventriculi. Ist ASS 100 trotzdem sinnvoll?
