Anlage eines zentralen Venenkatheters

Zusammenfassung

Der zentrale Venenkatheter ist ein Standardgefäßzugang bei der Versorgung von kritisch kranken Patienten. Er kann zur Gabe von Medikamenten und Ernährung, zum hämodynamischen Monitoring oder, in Form großlumiger Shaldonkatheter, als Dialysezugang verwendet werden.

Unter Berücksichtigung praktikabler Arbeitstechniken aus dem klinischen Alltag wird „Schritt für Schritt“ die Anlage eines ZVK beschrieben. Darüber hinaus werden die Vor- und Nachteile der gängigen Punktionstechniken und Punktionsorte anhand der aktuellen Literatur diskutiert, sowie Indikationen und Kontraindikationen kritisch beleuchtet.

Abstract

The cannulation of a central vein is a standard acces to the vascular system of critically ill patients. It can be used for administration of medication and parenteral nutrition, haemodynamic monitoring as well as hemodialsis via Shaldon catheter.

The technique of implantation of a central venous catheter is described step by step in this article. Moreover, advantages and disadvantages of the different techniques and puncture sites as well as indications and contraindications are critically discussed regarding the most recent literature.
