Summary

Case study report of 1.107 patients, 90% of which had lung puncture; the remainder had diseases of pleura, mediastinum and chest wall. The special dilating needle produces optimum tissue specimens, even from disseminated lung processes. Tissue cylinders 1.5 mm thick by up to 20 mm long can be examined histologically. This paper describes the instrumentarium and the technique. The complications are discussed, they depend upon morphology and topography of the findings; a fatal case, in extended pulmonary fibrosis and vicarious emphysema, because of late occurring pneumothorax; bronchoscopic aspiration was necessary in three severe hemorrhages; one case of transmission of the tumor into the puncture canal occurred 10 weeks after puncture; at one point, evidence of an air embolus was observed 36 hours after the puncture. The prerequisites for controlling of possible complications must be readily available. The diagnostic reliability of lung biopsy with the dilating needle is over 90%.
