Zusammenfassung

Hintergrund: Das eher seltene MALT-Lymphom (mucosa-associated lymphoid-like tissue lymphoma) zählt zu der niedrigmalignen Gruppe der Non-Hodgkin-Lymphome (NHL). Es manifestiert sich zu ca. 8 % in der Bindehaut. Nicht selten wird ein MALT-Lymphom der Bindehaut auch als chronische Konjunktivitis fehldiagnostiziert. Es kann durch eine Bestrahlungsbehandlung in vielen Fällen geheilt werden und hat eine günstige Prognose. Patient und Befund: Eine 61-jährige Patientin stellte sich in unserer Ambulanz wegen einer Veränderung der Bindehaut und einer Verschlechterung ihres Allgemeinzustandes vor. Die, von der Patientin beschriebenen, Symptome umfassten insbesondere Beschwerden wie Müdigkeit, Abgeschlagenheit, Nachtschweiß, Appetitlosigkeit und gastrointestinale Probleme. In der Anamnese findet sich ein vor 4 Jahren in sano reseziertes MALT-Lymphom der Bindehaut und ein vor 3 Jahren in sano reseziertes Rezidiv. Das Staging zu beiden Zeitpunkten ergab keinen Hinweis für Metastasen. Bei der ophthalmologischen Untersuchung zeigte sich eine nicht verschiebliche Prominenz der Bindehaut des rechten Auges bei 10 – 11 Uhr mit mäßiger konjunktivaler Injektion. Dies entsprach der Lokalisation des Ersttumors 2002 und des ersten Rezidivs 2003. Alle weiteren spaltlampenmikroskopischen und fundoskopischen Untersuchungen zeigten neben einer beidseitigen Cataracta incipiens keine Auffälligkeiten. Klinischer Verlauf: Zur Diagnosesicherung wurde eine Probeexzision der Bindehaut durchgeführt. Die histopathologische Begutachtung ergab ein Rezidiv des bekannten MALT-Lymphoms. Das Staging lieferte keinen Hinweis auf eine Metastasierung. Die Patientin wurde zur Einleitung einer Chemotherapie nach dem R-CHOP-Schema in der Onkologie vorgestellt. Schlussfolgerungen: Der von uns dargestellte Fall demonstriert, dass trotz eines in sano resezierten Befundes eine wiederholte Rezidivbildung des MALT-Lymphoms vorkommen kann. Liegen unspezifische Beschwerden bei Patienten mit einer Tumoranamnese vor, kann trotz makroskopisch unverändertem Lokalbefund erwogen werden, eine Probeexzision ex juvantibus durchzuführen.

Abstract

Background: The MALT lymphoma (mucosa-associated lymphoid-like tissue lymphomas) is a rare entity and belongs to the low-grade non-Hodgkin (NHL) lymphomas. In 8 % of cases it arises in the conjunctiva. In some cases a MALT lymphoma of the conjunctiva is misdiagnosed as chronic conjunctivitis. Mostly a MALT lymphoma of the conjunctiva can be cured by radiation and has a good prognosis. Case Report: A 61-year-old female presented to our hospital because of alteration of the conjunctiva and worsening of her general condition. Symptoms were fatigue, lassitude, night sweat, loss of appetite and gastrointestinal problems. The medical history revealed treatment for a MALT lymphoma 4 years previously and a relapse 3 years previously. Both were resected totally. Staging at those times gave no hint for metastases. The ophthalmological examination showed an adherent prominence with conjunctival injection of the right eye at 10 – 11 o’clock. It corresponded to the localisation of the primary tumour in 2002 and the relapse in 2003. Slit lamp and fundoscopic examinations only revealed a cataracta incipiens. Clinical Course: To confirm the diagnosis a biopsy was done. The histological examination demonstrated a relapse of the MALT lymphoma. Staging gave no hint for metastases. The patient was referred to the oncological unit for chemotherapy (R-CHOP regime). Conclusion: This case shows that a relapse of the MALT lymphoma may arise although the previous tumour and its relapse were resected totally. In patients with tumours in their medical history suffering from unspecific discomfort, a biopsy should may be be considered despite the lack of apparent macroscopic findings.
