Zusammenfassung

Einleitung: Die Behandlung der exsudativen altersabhängigen Makuladegeneration (AMD) mit VEGF-Inhibitoren hat sich aufgrund multizentrischer randomisierter Studien in den letzten Monaten immer mehr als Standardtherapie durchgesetzt. Auch die Sonderform der exsudativen AMD, die mit einer chorioidalen Neovaskularisation (CNV) assoziierte seröse Pigmentepithelabhebung (PED), kann hiermit abgeflacht werden. Sowohl im natürlichen Verlauf als auch nach der Behandlung durch Lasertherapie oder mit photodynamischer Therapie (PDT) waren die Ergebnisse wenig zufriedenstellend. Der Hauptgrund für eine fortschreitende Visusabnahme ist ein häufiges Rezidiv der CNV und ein Einriss des retinalen Pigmentepithels. Vorgestellt werden in dieser Arbeit die retrospektiv erhobenen Daten über die Möglichkeiten eines Wechsels des intravitreal applizierten Medikamentes bei ausbleibendem Behandlungserfolg. Methode: Es wird konsekutiv über 15 Patienten mit einer serösen PED berichtet, bei der nach funktionellem und morphologischem Therapieversagen nach Behandlung mit Pegaptanib (Macugen) die Therapie auf Lucentis umgestellt wurde. Sieben Patienten hatten eine seröse PED assoziiert an eine retinale angiomatöse Proliferation (RAP) und 8 Patienten eine seröse PED assoziiert an eine okkulte CNV. Das mittlere Alter betrug 74,67 Jahre. Der bestkorrigierte Visus wurde in LogMAR erhoben und zusätzliche morphologische Parameter waren die fovea full thickness in der optischen Kohärenztomografie (OCT), die manuelle Messung der PED-Höhe sowie die Angiografie mit den Farbstoffen Fluorescein (FAG) und Indozyaningrün (ICG). Bei funktioneller und morphologischer Verschlechterung des Befundes wurde von einer Therapie mit 1,25 mg Pegaptanib auf 0,5 mg Ranibizumab (Lucentis) umgestellt. Ergebnisse: Wir konnten ein mittleres Follow-up von 46,5 Wochen (95 % KI 35,9 – 57,1) der Analyse zugrunde legen. In keinem Fall kam es zu einem RIP (Riss im Pigmentepithel). Nach dem Wechsel des intravitrealen Medikamentes konnte eine Stabilisierung der Sehschärfe beibehalten werden. In 11 Fällen konnte eine Stabilisierung oder Verbesserung der Sehkraft nach dem Wechsel erreicht werden. Auch die Höhe der PED, die zuvor unter Pegaptanib (Macugen) unverändert blieb, nahm unter der Therapie mit Ranibizumab (Lucentis) ab. Einen Unterschied zwischen der serösen, assoziiert mit einer okkulten CNV, und einer RAP konnte nicht gesehen werden. Schlussfolgerungen: Bei der Behandlung der exsudativen AMD mit assoziierter seröser PED zeigt die Therapie mit VEGF-Inhibitoren hinsichtlich der Entwicklung der Sehschärfe und der morphologischen Parameter wie Netzhautdicke und Höhe der PED eine Wirksamkeit. Bei einem Therapieversagen nach Behandlung mit Pegaptanib konnten nach der Umstellung der Therapie auf Ranibizumab (Lucentis) funktionell stabile und morphologische verbesserte Befunde erreicht werden. Studien mit größeren Patientenzahlen sind zur weiteren Fundierung einer klinischen Effektivität notwendig.

Abstract

Introduction: Based on data from multicentric studies the treatment of exudative age-related macular degeneration (AMD) with VEGF inhibitors has recently become a more and more standard procedure. Nevertheless, the proper end point of the treatment and also the adequate interval between single injections have not been defined yet. In subtypes of AMD associated with pigment epithelium detachment. (PED) the natural course as well as laser coagulation treatment and photodynamic therapy (PDT) showed unsatisfactory results. The main cause of decreased visual acuity has proven to be recurrence of choroidal neovascularisation and ruptures of the pigment epithelium (RIP). This retrospective study presents the clinical results following a change of the injected VEGF inhibitor after ineffective initial therapy. Method: 15 patients with serous PED, 7 patients with PED associated with retinal angiomatous proliferation (RAP) exhibited insufficient stabilisation after initial treatment with pegaptanib (Macugen). Therefore we continued treatment with ranibizumab (Lucentis). The mean age was 74.67 years. Best corrected visual acuity was evaluated with logMAR. Additional morphological parameters were fovea full thickness (OCT), measurement of the height of pigment epithelium detachment and finally fluorescein and indocyaningreen angiography. In cases of functional and morphological deterioration, we changed the treatment from 1.25 mg pegaptanib (Macugen) to 0.5 mg ranbizumab (Lucentis). Results: During the mean follow-up of 46.5 weeks (95 % CI 35.9 – 57.1) no RIP occurred. After changing the drug, visual acuity showed a stabilisation or improvement in 11 cases. Also the height of PED decreased after beginning with ranbizumab. Regarding the therapeutic outcome, no difference between eyes with serous PED caused by occult CNV or RAP could be observed. Conclusion: Treatment using VEGF inhibitors in patients suffering from AMD with exudative serious PED has proven to be effective regarding both the morphological damage and functional injuries. In cases of therapeutic failure after pegaptanib functional and/or morphological stabilisation of the course of this disease can be achieved by changing to ranbizumab. Nevertheless, prospective clinical studies with a larger number of patients are necessary for confirmation of these findings.
