Zusammenfassung

Hintergrund: Die chronische Uveitis im Kindesalter ist eine häufige Komplikation bei der juvenilen idiopathischen Arthritis (JIA), die nicht selten zum Visusverlust führt. Neben den Kortikosteroiden und systemischen Immunsuppressiva werden zunehmend häufig die TNF-α-Inhibitoren eingesetzt. Material und Methoden: Übersicht der bis September 2006 publizierten Literatur und zusammenfassende Beurteilung der Wirksamkeit von TNF-α-Inhibitoren (Etanercept, Infliximab, Adalimumab) zur Behandlung der JIA-assoziierten Uveitis. Ergebnisse: TNF-α-Inhibitoren können erfolgreich zur Behandlung der chronischen Uveitis bei Kindern eingesetzt werden. Die Ansprechrate der Uveitis auf Etanercept betrug etwa 50 %, aber es traten sowohl weitere als auch erste Schübe und neue Komplikationen auf. Infliximab und Adalimumab scheinen zur Behandlung von Uveitis besser geeignet als Etanercept. Schlussfolgerungen: Die Therapie mit TNF-α-Inhibitoren ist teuer und erhöht das Langzeitrisiko für Zweiterkrankungen, wie z. B. für Tuberkulose und möglicherweise für Lymphome. TNF-α-Inhibitoren sollten nur bei therapierefraktärer aktiver Uveitis im Kindesalter eingesetzt werden, deren Verlauf durch Kortikosteroide und mindestens eines der etablierten Immunsuppressiva nicht beeinflusst werden konnte.

Abstract

Background: Chronic uveitis in childhood is a common complication of juvenile idiopathic arthritis (JIA) that frequently leads to loss of vision. Besides from corticosteroids and immunosuppressive drugs, Tumour necrosis factor-alpha (TNF-α) inhibitors are used frequently. Materials and Methods: The literature published before September 2006 was evaluated for the usefulness of TNF-α inhibitors (etanercept, infliximab, adalimumab) for the treatment of JIA-associated uveitis. Results: TNF-α inhibitors are effective drugs for the treatment of chronic uveitis in childhood. The response rate of uveitis in childhood to etanercept was approximately 50 %. However, disease recurrence, first manifestations of uveitis and new complications occurred during the treatment. Infliximab and adalimumab appear to be more effective for the treatment of uveitis in childhood than etanercept. Conclusions: The therapy with TNF-α inhibitors is expensive and increases the long-term risk for secondary diseases, such as tuberculosis and probably malignant lymphoma. Their use should be restricted to uveitis patients not responding to corticosteroids and at least one of established immunosuppressive drugs.
