Innovation statt Imitation - Neuigkeiten in der Arzneimitteltherapie

Die Zahl neu zugelassener Fertigarzneimittel, die einen neuen Wirkstoff enthielten, hatte 1997 mit 41 einen Höhepunkt erreicht. Seitdem geht in Deutschland die Markteinführung solcher Pharmaka zurück. Im Jahr 2003 wurden nur noch 17 neue Arzneistoffe zugelassen, davon sind sieben Substanzen als wirklich innovativ zu klassifizieren [15] [16]- eine Entwicklung, wie sie auch weltweit zu beobachten ist [25]. Bewährt hat sich dabei die Einteilung von Fricke [15], der folgende Kriterien zugrunde liegen:

A: innovative Struktur bzw. neuartiges Wirkprinzip mit therapeutischer Relevanz,

B: Verbesserung pharmakodynamischer oder pharmakokinetischer Eigenschaften bereits bekannter Wirkprinzipien,

C: Analogpräparate ohne oder mit nur marginalen Unterschieden zu bereits eingeführten Präparaten,

D: nicht ausreichend gesichertes Wirkprinzip oder unklarer therapeutischer Stellenwert.

Tab. [1] stellt einen Auszug der neuen Wirkstoffe/Wirkprinzipien der Jahre 2002 und 2003 dar. Man sieht, dass in den letzten Jahren immer wieder Arzneimittel entwickelt wurden, die einen klaren Fortschritt für die Arzneitherapie bedeuten. Blickt man auf die Forschungs-Pipelines der pharmazeutischen Unternehmen, dürfen wir auch in naher Zukunft auf solche Produkte hoffen.

Beispiele von tatsächlichen Innovationen, die zum Teil schon zugelassen sind oder kurz vor der Zulassung stehen werden im Folgenden unter Berücksichtigung ihrer pharmakologisch-therapeutischen Eigenschaften und der wichtigsten klinischen Studien kurz dargestellt. Bei den ausgewählten Substanzen handelt es sich um eine naturgemäß stark selektierte Auswahl von neuartigen Wirkstoffen oder innovativen Wirkprinzipien mit großer therapeutischer Relevanz für eine Vielzahl von Patienten.
