Postalimentäre Hypoglykämien bei Spätdumping-Syndrom

Zusammenfassung

Anamnese und klinischer Befund: Bei einem 48-jährigen Patienten kam es seit Jahren zu postprandialen Hypoglykämien mit Unruhe, Zittern, Schweißausbrüchen und Heißhunger, regelhaft etwa 2 Stunden nach Nahrungsaufnahme. 5 Jahre zuvor waren mehrmals eine Fundoplicatio sowie eine selektiv proximale Vagotomie aufgrund einer Refluxösophagitis durchgeführt worden.

Untersuchungen: Die körperliche Untersuchung ergab keine relevanten pathologischen Befunde. Im Blutzuckertagesprofil mit stündlicher Kapillarblutzuckermessung zeigten sich eine physiologische Nüchtern-Blutzucker(BZ)-Homöostase und jeweils 2 Stunden postprandial BZ-Abfälle bis auf 20 mg/dl. Im oralen Glukosebelastungstest ließ sich ein auffälliges Insulinsekretionsverhalten mit einem pathologisch erhöhten Insulin/Glukose-Index dokumentieren. Szintigraphisch zeigte sich eine zunächst verzögerte, dann stark beschleunigte Magenentleerung als Folge der selektiv proximalen Vagotomie.

Diagnose, Therapie und Verlauf: Es wurde die Diagnose post-alimentäre Hypoglykämien durch Insulinhypersekretion im Rahmen eines postoperativen Spätdumping-Syndroms gestellt. Der Versuch einer operativen Pyloroplastik erbrachte keinen Erfolg. Neben einer Änderung der Essgewohnheiten erfolgte eine Behandlung mit subkutan appliziertem Octreotid (Sandostatin®), einem Somatostatin-Analogon.

Folgerungen: Postalimentäre Hypoglykämien sind oft bereits durch eine sorgfältige Anamneseerhebung einem Spätdumping-Syndrom zuzuordnen. Letzte Sicherheit kann durch einen oralen Glukose-Provokationstest mit Erfassung der Insulinsekretion und eine Magenentleerungsszintigraphie gewonnen werden. Neben anderen Therapiemöglichkeiten ist die Behandlung mit Octreotid eine Erfolg versprechende und nebenwirkungsarme Alternative.

Summary

History and clinical findings: A 48-year-old patient had been suffering from postalimentary hypoglycemias for several months, occurring regularly 2 hours after a meal. 5 years before, repeated fundoplications as well as a selective proximal vagotomy due to reflux oesophagitis had been performed.

Investigations: Physical examination revealed no pathological findings. The diurnal blood sugar profile with hourly capillary glucose measurement showed a physiological fasting glucose homeostasis and two-hour postprandial decrease of blood glucose down to 20 mg/dl. The oral glucose tolerance test revealed a noticeable insulin secretion with a pathologically increased insulin/glucose index. Scintigraphy demonstrated an initially delayed, then accelerated gastric emptying as a consequence of the selective proximal vagotomy.

Diagnosis, treatment and course: A postalimentary hypoglycemia by hypersecretion of insulin in the context of a post-gastrectomy late dumping syndrome was diagnosed. A surgical pyloroplasty was not effective. In addition to the modification of eating habits, treatment with subcutaneous applicated octreotide (Sandostatin®), a somatostatin-analogue, was initiated.

Conclusions: Postalimentary hypoglycemia can be assigned to late dumping syndrome in most cases already by ascribed history taking. The correct diagnosis can be achieved by an oral glucose tolerance test with measurement of insulin secretion and gastric emptying scintigraphy. Beside other therapeutical options the treatment with octreotide is a promising alternative with manageable side effects.
