Akute Ischämie nach intraarterieller Drogeninjektion

Anamnese und klinischer Befund: Ein 36-jähriger Patient stellte sich mit einer schweren Handischämie nach intraarterieller Injektion von vier in Wasser aufgelösten Tabletten à 10 mg Methylphenidat (Ritalin^®) in die rechte Arteria radialis vor.

Untersuchungen: Die nicht invasive angiologische Diagnostik ergab eine unauffällige Perfusion der Arteria radialis und ulnaris, jedoch eine schwere Perfusionsstörung der Fingerarterien der rechten Hand.

Therapie und Verlauf: Bei Verdacht auf kombinierte vasospastisch und thromboembolisch bedingte Gefäßverschlüsse führten wir eine intraarterielle Lyse mit Urokinase sowie anschließend eine Vollheparinisierung durch und applizierten Prostaglandininfusionen. Trotz dieser Maßnahmen kam es zu Nekrosen im Bereich der Mittel- und Endglieder dreier Finger; nach Teilamputation der Finger war die Wundheilung unkompliziert.

Folgerung: Eine Extremitätenischämie nach intraarterieller Injektion von Drogen und Medikamenten ist ein angiologischer Notfall.

History and clinical findings: A 36-year-old patient presented with a severe hand ischemia after intraarterial injection of four dissolved tablets of Methylphenidate each 10 mg (Ritalin^®) into the right radial artery.

Investigations: Non-invasive vascular diagnostic demonstrated a normal perfusion of the radial and ulnar artery and a severe ischemia of the small vessels of the right hand.

Treatment and course: Under suspicion of a combined vaso-spastic and thromboembolic arterial occlusion we started an intraarterial lysis therapy followed by anticoagulation with heparin and infusions of prostaglandines. Despite this therapy necroses of three fingers developed; partial amputation was followed by a good wound healing.

Conclusions: Ischemia of an extremity after intraarterial injection of drugs is a vascular emergency.
