Spondyloarthritiden - neue Aspekte zur Klassifikation, Diagnostik und Therapie

Die Spondyloarthritiden (SpA) zählen neben der rheumatoiden Arthritis zu den wichtigsten und häufigsten entzündlich-rheumatischen Erkrankungen. Zu den SpA werden die ankylosierende Spondylitis (AS, Morbus Bechterew), die reaktive Arthritis (ReA, Morbus Reiter), die Arthritis psoriatica, die SpA bei chronisch entzündlichen Darmerkrankungen, der spondyloarthritische Typ der juvenilen chronischen Arthritis und die so genannten undifferenzierten SpA gerechnet. Den SpA sind neben typischen rheumatologischen Befallsmustern spezifische extraskelettale Manifestationen, eine familiäre Häufung und ähnliche immungenetische Pathomechanismen gemeinsam. Obwohl die Prävalenz der SpA nach jüngsten epidemiologischen Untersuchungen in Deutschland bei ca. 2 % liegt [8], muss von großen Defiziten des Wissens über frühe pathognomonische Symptome, Frühdiagnostik, prognostische Faktoren und sozioökonomische Bedeutung dieser Krankheitsgruppe ausgegangen werden. So werden die klinischen Leitsymptome des entzündlichen Rückenschmerzes mit Nachtschmerz und Morgensteifigkeit sowie der Mono- oder Oligoarthritis bei jüngeren Menschen nicht selten fehlgedeutet; die primäre Betreuung und Diagnostik dieser Patienten erfolgt meist nicht durch rheumatologische Fachärzte. In Deutschland liegen derzeit noch immer 5 - 7 Jahre zwischen ersten typischen Symptomen und der Erstdiagnose einer AS [48].
