Aortenklappeninsuffizienz: Wann ist der optimale Operationszeitpunkt ?

Ein Aortenklappenersatz kann heutzutage mit großer technischer Sicherheit unter dem Einsatz der Herz-Lungen Maschine durchgeführt werden. In den letzten 30 Jahren ist es dabei zu einer stetigen Verbesserung nicht nur der operativen Technik, der Klappensubstitute, sondern auch der perioperativen Intensivbetreuung gekommen. Dies hat zu einer stetigen Ausdehnung der operativen Indikationsstellung, insbesondere im Alter geführt. Gegen diese weitreichenden operativen Möglichkeiten muss die konservative Therapie abgewogen werden. Damit kommt der Abstimmung der Zeitdauer der konservativen Therapie und Planung des operativen Zeitpunktes im Einzelfall eine besondere Bedeutung zu, um den idealen Operationszeitpunkt nicht zu verpassen. Die Aortenklappeninsuffizienz ist das Klappenvitium, bei dem die Abwägung zwischen beiden Therapiealternativen hinsichtlich der zeitlichen Planung besonders schwierig ist, so dass dieser Übersichtsartikel versucht, den optimalen Operationszeitpunkt auf der Basis der Literatur zusammen zu fassen.
