Ventilatorische und phonatorische Atmungsfunktion

Zusammenfassung

Nach kurzem Überblick über die funktionell-anatomischen Grundlagen des Atmungsorganes wird die ventilatorische und phonatorische Atmungsfunktion erklärt. Physiologisch richtig ist die kostoabdominale Atmung, wobei Bauch- und Brustwandbewegungen der sog. Relaxationslinie folgen sollen, d. h. den Positionsverhältnissen von Thorax und Abdomen, die sich bei verschiedenen Lungenvolumina nach vollständiger Entspannung gegen die geschlossene Glottis einstellen. Sowohl die Ruhe- als auch die Phonationsatmung bewegt sich um die Atemruhelage herum, da sich in dieser Entspannungslage der Atmungsapparat mit dem geringsten Energieaufwand bewegen lässt. Die neuromuskuläre Atmungskontrolle folgt diesem physiologischen Prinzip des energetischen Minimalaufwandes, indem sie Muskelarbeit und elastische Rückstellkräfte in regelmäßigem Wechsel für den Atmungszyklus nutzt. Jedes Abweichen von Entspannungslinie und Atemruhelage bedeutet zusätzliche muskuläre Arbeit. Zentrale Bedeutung für die Stimmgebung - insbesondere beim Singen - hat die Atemstütze („Apoggio”), d. h. die gezielte inspiratorische Gegenaktivität gegen die Ausatmungsbewegung zur Kontrolle des subglottischen Anblasedruckes. Weder das Zwerchfell noch eine andere gesonderte Muskelgruppe ist allein verantwortlich für die Atemstütze. Vielmehr läßt sich bei der Phonationsatmung pneumographisch und elektrophysiologisch ein subtiles Zusammenspiel inspiratorischer und exspiratorischer Kräfte nachweisen, das nicht nur von Körperhaltung und Phonationsart, sondern auch vom individuellen Trainingszustand der Stimme abhängt. Funktionelle Atmungsfehler sind Bewegungsabläufe bei Ruhe- und Phonationsatmung, die nicht den physiologischen Richtlinien folgen. Ihre genaue Analyse ist Voraussetzung für eine gezielte Atmungstherapie.

The article gives a short review of the anatomical basics of the respiratory apparatus followed by a description of its function in ventilation and phonation. During the physiologically correct costo-abdominal breathing the movements of the chest and the abdominal wall follow the so-called relaxation line; i. e. their relative configurations at different lung volumes obtained by complete muscular relaxation against the closed glottis. At rest as well as during phonation the respiratory apparatus moves around its resting position (functional residual capacity) because in this relaxed position he respiratory system can be moved with a minimum of power. The neuromuscular breath control follows the physiological principle of minimum energetic expenditure by alternately using muscular activity and passive elastic recoil forces for the in-exspiratory breathing cycle. Each deviation from the relaxation line and the resting position causes additional muscular work. An important mechanism for the phonatory process, especially in singing, is the breathing support (”Apoggio”), i. e. the subtly tuned antagonism of inspiratory muscles against exspiratory recoil forces of lung and rib cage being necessary for a proper adjustment of the subglottic driving pressure. Neither the diaphragm nor any other individual muscle group has single responsibility for the breathing support. A precise synergism between inspiratory and exspiratory forces during phonation can shown pneumographically and electrophysiologically, depending on body position, kind of phonation and, especially, on the individual status of voice education. Functional breathing disorders are defined as respiratory patterns, both at rest and during phonation, being aberrant from physiological principles. The exact analysis of such disorders is fo crucial importance for a proper breath therapy.
