Arterial switch operation remains the gold standard surgical therapy for D-transposition
of the great vessels. Coronary anomalies may complicate the surgical strategy and
lead to an increased morbidity and mortality. However, as of today, even challenging
coronary anomalies should not be a contraindication for the performance of complete
repair. Here, we describe a technique for the translocation of an intramural, high-origin
conal branch in a 5-month-old patient with D-transposition of the great vessels and
ventricular septal defect.
