Zusammenfassung
Anamnese
Zwei Patientinnen stellten sich kurz nacheinander in unserer Klinik vor–mit Sehstörungen,
Sehen von Doppelbildern, Schluckstörungen und Luftnot, wenige Tage nach intragastraler
Injektion von Botulinumtoxin in einer Klinik in der Türkei.
Untersuchungsbefunde
Die körperliche Untersuchung war bis auf Hinweise auf eine respiratorische Insuffizienz
mit erhöhter Atemfrequenz und Luftnot vor allem im Liegen unauffällig, eindeutige
neurologische Symptome fanden sich nicht. In der Bildgebung fanden sich Hinweise auf
eine verminderte Zwerchfell-Beweglichkeit. Die Blutgas-Analysen zeigten eine hypoxämische
respiratorische Insuffizienz.
Verlauf
Die Patientinnen wurden ohne spezifische Therapie einige Tage lang stationär beobachtet
und mit rückläufiger Symptomatik entlassen.
Folgerung
Im März 2023 veröffentlichten das Robert-Koch-Institut, das European Centre for Disease
Prevention and Control und die WHO einen Bericht über 87 Patienten, die in 2 türkischen
Krankenhäusern mit dem Ziel einer Gewichtsreduktion intragastrale Injektionen von
Botulinumtoxin erhalten und nachfolgend Symptome eines Botulismus entwickelt hatten.
Die beiden hier vorgestellten Fälle verdeutlichen die Problematik der weiterhin durchgeführten
Prozedur und die Risiken eines Botulismus.
Abstract
History
Two female patients presented to our clinic shortly after one another with visual
disturbances, double vision, swallowing difficulties and shortness of breath, a few
days after an intragastric injection of botulinum toxin in a clinic in Turkey.
Clinical findings
The physical examination was unremarkable except for signs of respiratory insufficiency
with increased respiratory rate and shortness of breath, especially when lying down;
no clear neurological symptoms were found. Imaging showed reduced diaphragmatic mobility.
Blood gas analyses showed hypoxemic respiratory insufficiency.
Course
The patients were observed as inpatients for a few days without specific therapy and
were discharged with regressing symptoms.
Conclusion
In March 2023, the Robert Koch Institute, the European Centre for Disease Prevention
and Control and the WHO published a report on 87 patients who had received intragastric
injections of botulinum toxin in 2 Turkish hospitals with the aim of losing weight
and subsequently have developed symptoms of botulism. The current cases presented
here draw further attention to the problems of the procedure that is still being carried
out and to the risks of botulism.
