Zusammenfassung

Die letzten 30 Jahre waren in der Proktologie eine äußerst innovative Zeit, in der proktologische Leiden mit neuen Therapien und Methoden behandelt wurden. Allerdings mussten sich diese Innovationen einer wissenschaftlichen Evaluation stellen und konnten in einigen Fällen den Erwartungen nicht vollständig gerecht werden. Besonders die Reproduzierbarkeit der Ergebnisse durch andere Anwender entsprach oft nicht den vielversprechenden Resultaten der Erstautoren. Beim Hämorrhoidalleiden wurde v.a. versucht, die schmerzhaften Folgen der klassischen Hämorrhoidektomie zu vermeiden. Diskutiert werden der anale Lift, die Laserhämorrhoidoplastie, die Radiowellentherapie und die Embolisation der Aa. haemorrhoidales.

Im Kontext des Fistelleidens ist die Schonung des Sphinkterapparats von höchster Bedeutung, während gleichzeitig ein Rezidiv oder eine Persistenz der Fistel vermieden werden soll. Hier kommen innovative Methoden wie der Fistelplug, die LIFT-Technik, die Lasertherapie sowie die Stammzelltherapie zum Einsatz, die im Folgenden kritisch gewürdigt werden.

Ein besonderes Augenmerk liegt auf der Therapie der Stuhlinkontinenz. Leider haben sich einige vielversprechende Ansätze, wie der künstliche Darmsphinkter, das Magnetband und die Radiofrequenztherapie, als wenig erfolgreich erwiesen. Fortschritte wurden in der Behandlung HPV-bedingter Erkrankungen erzielt. Aktuelle Studien belegen die Relevanz einer HPV-bedingten Dysplasie bei der Entwicklung eines Analkarzinoms, dies unterstreicht das notwendige Screening durch die innovative HRA (High Resolution Anoskopie).

Als Innovation wird auch immer wieder die Materialität der proktologischen Untersuchungsgeräte bezeichnet. Die Entwicklung zu Single-use-Geräten ist unverkennbar, begründet mit Hygiene- und Kostenaspekten. Diese Sichtweise blendet die Nachhaltigkeit sowie die Schadstoffbilanz durch Kunststoffe und in den Geräten verbaute Batterien und LEDs leider vollkommen aus.

Abstract

The past 30 years have been an extremely innovative period in proctology, during which proctological conditions have been addressed with new therapies and methods. However, the innovations had to undergo scientific evaluation and in some cases did not meet the requirements of reality. In particular, the reproducibility by others was not given, in contrast to the promising results of the first authors.

In the case of haemorrhoidal disease, the main aim was to avoid the painful consequences of the classic haemorrhoidectomy. The anal lift, laser haemorrhoidoplasty, radio frequency therapy and embolization of the haemorrhoidal arteries are discussed. In the context of fistula disease, it is extremely important to protect the sphincter apparatus while at the same time avoiding recurrence or persistence of the fistula. Fistula plug, LIFT technique, laser therapy and stem cell therapy are the innovative methods that will be critically evaluated below. The treatment of fecal incontinence is of great importance. Unfortunately, many innovative approaches such as the artificial bowel sphincter, magnetic sling and radio frequency therapy have been unsuccessful. And very recently, good studies have shown the relevance of HPV-related dysplasia in the development of anal carcinoma and the necessary screening using HRA (high resolution anoscopy).

The materiality of proctological examination devices is also repeatedly described as an innovation. The trend towards single-use devices is obvious, based on hygiene and cost aspects. Unfortunately, this view completely ignores sustainability and the pollutant balance caused by plastics and the batteries and LEDs used in the devices.
