Erfolgreiche Kieferorthopädische Einstellung von ektopen unteren Eckzähnen – 3 Fallberichte
Successful orthodontic alignment of ectopic lower canines – 3 Case Reports

Hintergrund
Ektopische untere Eckzähne kommen relativ selten vor, stellen für den betroffenen Patienten und Behandler aber eine Herausforderung dar, eine adäquate Therapie zu finden. Zwischen einer Extraktion mit Lückenschluss, (implantat)prothetischer Versorgung oder einer kombiniert chirurgisch-kieferorthopädischen Einordnung kann im Wesentlichen entschieden werden.
Fallvorstellungen
Bei zwei jugendlichen Patienten mit einem verlagerten Eckzahn und einem jugendlichen Patienten mit 2 verlagerten Eckzähnen erfolgte im Rahmen einer festsitzenden kieferorthopädischen Multibrackettherapie die Einordnung der Eckzähne nach chirurgischer Freilegung und Traktion mittels des EWC-Systems (PSM Medical Solutions).
Schlussfolgerungen
In allen Fallberichten konnten unter Anwendung des EWC-Systems und Lingualbögen als Verankerung die verlagerten Eckzähne eingestellt werden. Trotz der relativ langen Behandlungszeit stellt dieses Vorgehen bei richtiger biomechanischer Planung und Durchführung eine zahnerhaltende Therapieoption dar.
Abstract
Background
Ectopic lower canines are relatively rare but present a significant challenge for both the patient and treating clinician in determining and appropriate therapeutic approach. The primary treatment options include extraction with space closure, (implant-supported) prosthetic rehabilitation, or a combined surgical-orthodontic repositioning.
Case Presentations
In two adolescent patients with a single impacted canine and one adolescent patient with two impacted canines, orthodontic alignment was performed as part of a fixed braces treatment. Following surgical exposure, the canines were guided into their correct position using the EWC system (PSM Medical Solutions) with traction.
Conclusions
In all reported cases, the use of the EWC system in combination with lingual arches for anchorage successfully facilitated the alignment of the impacted canines. Despite the relatively long treatment duration, this approach represents a tooth-preserving treatment option when planned and executed with appropriate biomechanical principles.
