Noncompliance: „Kontraindikation” für die Organtransplantation?

Zusammenfassung

Noncompliance wird häufig als „Kontraindikation” für die Durchführung einer Organtransplantation bezeichnet. Dies ist zunächst verständlich, ist doch die regelmäßige Einnahme von Immunsuppressiva im Anschluss an die Transplantation Voraussetzung für den dauerhaften Erfolg der Behandlung. Dennoch erscheint es aus ethischer Sicht unangemessen, Noncompliance als Kontraindikation zu klassifizieren. Erstens besteht so die Gefahr, dass vielfältige Möglichkeiten der Beeinflussung des Patientenverhaltens zu schnell aus dem Blick geraten. Zweitens kann diese Klassifizierung auf für den Patienten nachteilige Weise dazu führen, dass Ärzte sich eher als neutrale Beobachter und Beurteiler denn als teilnehmende Berater und Unterstützer des Patienten verstehen. Dem sich daraus ergebenden besonderen Status des Noncompliance-Kriteriums sollte in der klinischen Praxis und auch in ärztlichen Richtlinien zur Organtransplantation Rechnung getragen werden.

Summary

Noncompliant behaviour is often taken to constitute a „contraindication” to an otherwise necessary organ transplantation. At the face of it this appears reasonable, given that strict adherence to the regimen of immunosuppressive medication following the transplantation is a prerequisite of the therapy’s success. Nonetheless, from an ethical point of view the classification as contraindication is questionable. Firstly, doing so may lead to lose sight of the manifold possibilities to change patient behaviour. Secondly, the classification wrongly suggests that the physician ought to relate to the patient as a neutral observer and judge rather than as a participative guide and assistant. Clinical practice as well as medical guidelines on organ transplantation ought to take account of this special status of the criterion of non-compliance.
