Multiple Intestinal Duplications in a Child with Thoracic Myelomeningocele and Hydrocephalus

Abstract
An infant with thoracic spina bifida, myelomeningocele and hydrocephalus was found to have intrathoracic gastric duplication and noncontiguous tubular duplication along two-thirds of the small bowel. The myelomeningocele was closed and the hydrocephalus relieved with a shunt. The intrathoracic duplication was excised in toto and the intra-abdominal malformation successfully treated by stripping the entire mucosal tube from within the duplication.
Zusammenfassung
Multiple intestinale Duplikaturen sind seltene, angeborene Mißbildungen unbekannten Ursprungs. Man weiß, daß intestinale Duplikaturen gleichzeitig mit Anomalien der tieferen Hals- und höheren Brustwirbelsäule vorkommen. Wir berichten von einem Mädchen mit sagittalen Mittelliniendefekten im 1 .-5. Brustwirbelkörper, das außerdem eine ungewöhnliche Kombination anderer kongenitaler Anomalien aufwies. Anfangs wurde eine thorakale Myelomeningozele mit gleichzeitigem Hydrozephalus behandelt. Später wurde eine intrathorakale Magen-Duplikatur und eine weitere von ihr unabhängige, langstreckige röhrenförmige Darm-Duplikatur festgestellt. Die Myelomeningozele wurde verschlossen und der Hydrozephalus mit einem Shunt versorgt. Die intrathorakale Duplikatur wurde vollständig exzidiert und die Dünndarm-Duplikatur durch die Entfernung der gesamten Mukosa innerhalb der Duplikatur geheilt. Postoperative Komplikationen traten nicht auf.
