Perforated Nonspecific Ulcer of the Colon in Children

Abstract

Nonspecific ulceration of the colon is rarely seen in children. The most common location is the cecum, but the left colon is the involved segment in 15 % of patients.

We describe two affected children, who presented acutely with free colonic perforation. In the first patient, treatment consisted of exteriorization followed by delayed closure, and in the second child of localized excision followed by primary two-layer suture. Both patients recovered uneventfully.

Zusammenfassung

Unspezifische Ulzera des Kolons sind im Kindesalter selten. Die häufigste Lokalisation ist das Zökum, aber auch das linke Kolon ist bei 15 % der Patienten betroffen. In der vorliegenden Arbeit werden zwei Kinder mit einer akuten freien Kolonperforation vorgestellt. Eine hierfür verantwortliche Grunderkrankung konnte weder histologisch noch serologisch festgestellt werden. Bei dem ersten Patienten bestand die Behandlung in einer Vorverlagerung mit zweiseitigem Verschluß, bei dem zweiten Kind wurde die Perforationsstelle exzidiert und primär zweischichtig verschlossen. Unspezifische Ulzera sind im Kindesalter zwar selten, müssen jedoch bei der Beurteilung einer akuten Bauchsymptomatik immer mitbedacht werden.
