Zusammenfassung
Hintergrund: Das Auftreten von Schulterschmerz nach erlittenem Schlaganfall ist eine häufige Komplikation, die die motorische Erholung, Aktivität und die Lebensqualität der Betroffenen beeinträchtigt.
Ziel: Die Arbeit untersuchte die Frage nach der Evidenz physiotherapeutischer Maßnahmen zur Prävention und Behandlung von Schulterschmerz in der Schlaganfallrehabilitation.
Methode: Die Literatursuche in den medizinischen Datenbanken Medline, Cochrane und PEDro erzielte 1 Metaanalyse, 6 Reviews, 8 RCT und 1 CT.
Ergebnisse: Nur 1 von 7 Methoden zur Schmerzbewertung wies bei Schlaganfallpatienten die nötige Reliabilität und Validität auf. Studien zu funktioneller Elektrostimulation und transkutaner elektrischer Nervenstimulation ergaben widersprüchliche Ergebnisse bezüglich der Schmerzbehandlung im Gegensatz evidenzbasierten Effekten bei intramuskulärer Elektrostimulation. Einzelne Tapetechniken erzielten eine erfolgreiche Prävention von Schulterschmerz. Apparatives Durchbewegen zeigte eine Tendenz zur Schmerzreduktion, Dehnungslagerung erzielte keine Schmerzlinderung. Zu weiteren Behandlungsansätzen kann bisher keine evidenzbasierte Aussage getroffen werden.
Schlussfolgerungen: Elektrotherapie, Tapen der Schulter und endgradiges passives Bewegen scheinen wirkungsvolle Präventions- und Behandlungsmöglichkeiten von Schulterschmerz zu sein. Zu Bestätigung dieser Aussage sind weitere, qualitativ hochwertige Studien und Untersuchungen notwendig, die die Reliabilität und Validität von Messmethoden für Schmerz an Schlaganfallpatienten nachweisen.
Abstract
Background: Hemiplegic shoulder pain is a common post-stroke complication that reduces motor recovery, activity and quality of life of the person concerned.
Objective: This article investigated the question of the evidence of physiotherapeutic measures focussed on prevention and treatment of hemiplegic shoulder pain in stroke rehabilitation.
Methods: The literature search in the medical databases Medline, Cochrane and PEDro achieved 1 meta-analysis, 6 reviews, 8 RCT and 1 CT.
Results: Only 1 of the 7 methods applied to stroke patients in order to evaluate pain offered the required reliability and validity. Trials of functional electrical stimulation and transcutaneous electrical nerve stimulation showed contradictory results concerning pain treatment whereas intramuscular electrical stimulation gained evidence-based results. Specific taping techniques achieved successful prevention of hemiplegic shoulder pain. Continuous technical passive motion tended towards pain relief, static positional did not alleviate pain. An evidence-based statement concerning further treatment approaches is so far not possible.
Conclusions: Electrical stimulation, shoulder taping and end-range passive motion seem to be effective treatment modalities for hemiplegic shoulder pain. To verify this, high-quality clinical trials and studies are necessary to prove reliability and validity of measurements for hemiplegic shoulder pain in stroke patients.
