Early Predictors of Success of Kasai Operation in Children with Biliary Atresia

Abstract

Aim: Aim of the study was the evaluation of early predictive parameters of event‐free survival (not listed for liver transplantation, not transplanted, no death) in children suffering from biliary atresia after hepatoportoenterostomy (Kasai procedure) in order to optimize pretransplant management. Patients and Methods: Sixty-seven infants were treated with the Kasai operation at our institution over a 20‐year period from 1978 until 1998. Median age at time of operation was 51 days after birth (range 19 - 180 days). Of these 67 infants, 24 children with complete datasets and an observation time of at least one year were evaluated retrospectively using a Cox regression model. The response variable was event-free survival after a median observation time of 4.9 years (1.11‐ 10.37 years). Six variables were entered as covariates: alanine aminotransferase (ALAT), cholinesterase activity, bilirubin, age at the time of Kasai operation and tracer excretion and uptake during hepatobiliary scintigraphy (HBSS). All variables were evaluated six weeks after operation. For subsequent cut‐off determination, a receiver operating analysis (ROC analysis) was carried out. Results: Tracer excretion shown by HBSS showed the highest prognostic power to predict event-free survival after Kasai operation (log rank 18.68, p < 0.0001) followed by bilirubin and ALAT as further significant parameters in the first univariate step of the Cox regression model. In the subsequent multivariate step, the prognostic power of HBSS was improved only by bilirubin (log rank 24.6, p < 0.0001). The ROC analysis determined a cut‐off for bilirubin concentrations of 57 µmol/l for event‐free survival with a sensitivity of 80 % and a specificity of 78.6 %. The five-year event‐free survival‐rate was 100 % in the group with good tracer excretion and a bilirubin concentration of 57 µmol/l and 27 % for the other group (log rank test, p < 0. 0001). Conclusion: Early predictors of success of the Kasai operation in children with biliary atresia are free tracer excretion as shown by HBSS and a serum bilirubin concentration < 57 µmol/l six weeks after the operation. Thus, children with bilirubin concentrations above this level should be carefully and frequently monitored with regard to a transplantation requirement in order to optimize pretransplant management.

Facteur pronostique précoce de succès de l'opération de Kasai chez les enfants porteurs d'une atresie des voies biliaires

Buts: Le but de cette étude est l'évaluation des paramètres prédictifs précoces sans complication (non listés pour une transplantation hépatique, non transplantés, non décédés) chez des enfants souffrant d'une atrésie des voies biliaires après hépato-porto-entérostomie (intervention de Kasai) dans le but d'optimiser le management pré-transplantation. Patients et Méthodes: 67 enfants étaient traités avec l'opération de Kasai dans notre institution pendant une période de 20 ans de 1978 à 1998. L'âge médian au temps de l'opération était de 51 jours après la naissance (moyenne 19 à 180 jours). Parmi ces 67 enfants, 24 enfants avaient une observation complète et un temps de suivi d'au moins une année et étaient évalués rétrospectivement en utilisant le modèle de régression de Cox. La période moyenne d'observation sans complication était de 4,9 années (1,11 à 17,37). 6 variables étaient introduites: alanine aminotransferase (ALAT), activité cholinestérasique, bilirubine, âge au temps du Kasai, excrétion et fixation durant la scintigraphie hépatique (HBSS). Toutes les variables étaient évaluées 6 semaines après l'opération. Résultats: L'excrétion montrée par HBSS témoignait d'une possibilité pronostique pour prédire une évolution sans complication après l'opération de Kasai (log rank 18,68, p < 0,0001) suivie par la bilirubine et les ALAT. Le pouvoir pronostique de HBSS était amélioré seulement par le taux de bilirubine (log rank 24,6, p < 0,0001). L'analyse ROC déterminait une concentration de bilirubine limite à 57 µmol/l pour les patients survivants sans complication avec une sensibilité de 80 % et une spécificité de 78.6 %. Le taux de survie à 5 ans sans complication était de 100 % dans le groupe avec une bonne excrétion, une concentration de bilirubine de 57 µmol/l et 27 % pour l'autre groupe (log rank, p < 0,0001). Conclusion: Les facteurs pronostiques de succès de l'opération de Kasai chez l'enfant avec atrésie des voies biliaires sont l'excrétion montrée par HBSS et une concentration en bilirubine inférieure à 57 µmol/l 6 semaines après l'opération. Aussi, ces enfants avec des concentrations de bilirubine au-dessous de ce niveau devaient être suivis avec soin et fréquemment dans le but d'une éventuelle transplantation pour optimiser le management de pré-transplantation.

Predictores tempranos de éxito de la operación de Kasai en niños con atresia biliar

Objetivo: El objetivo de este estudio fue la evaluación de los parámetros predictivos precoces de sobrevivencia sin complicaciones (entrada en lista de trasplante, trasplante o fallecimiento) en niños con atresia biliar tras hepatoportoenterostomía de Kasai con el fin de optimizar el tratamiento pretrasplante. Pacientes y Métodos: Tratamos 67 pacientes con la operación de Kasai en el periodo de 20 años comprendido entre 1978 y 1998. La edad mediana en el momento de la operación fue 51 días tras el nacimiento (rango 19 - 180). 24 de estos 67 niños tenían documentación suficiente y un tiempo de observación de por lo menos un año por lo que fueron evaluados retrospectivamente según un modelo de regresión de Cox. La variable estudiada fue la sobrevivencia sin complicaciones tras una observación mediana de 4,9 años (1,11 - 10,37). Se consideraron 6 variables como covariantes: alanin-aminotransferasa (ALAT), colinesterasa, bilirrubina, edad en el momento del Kasai y captación y excreción de radiotrazador durante la escintigrafía hepatobiliar (HBSS). Todas las variables fueron estudiadas 6 semanas tras la intervención. Para determinar valores umbrales se utilizó un análisis de curva ROC. Resultados: La excreción de radiotrazador en el HBSS mostró el mejor valor pronóstico para predecir sobrevivencias sin complicaciones post-Kasai (Log rank 18,68, p < 0,0001) seguido de la bilirrubina y ALAT como parámetros significativos en el primer paso univariante del modelo de regresión de Cox. En el siguiente paso multivariante el valor pronóstico del HBSS fue mejorado solamente asociándolo a bilirrubina (Log rank 24,6, p < 0,0001). El análisis ROC determinó valores umbral para la concentración de bilirrubina de 57 µmol/L para una sobrevivencia sin complicaciones con una sensibilidad de 80 % y una especificidad de 78,6 %. La tasa de sobrevivencia sin complicaciones a 5 años en el grupo con buena excreción de trazador y bilirrubina < 57 µmol/L fue del 100 % y en el otro grupo del 27 % (Log rank, p < 0,0001). Conclusión: Los mejores predictores tempranos del éxito de la operación de Kasai en niños con atresia biliar son la excreción de trazador en el HBSS y la concentración de bilirrubina menor de 57 µmol/L 6 semanas tras la operación. Por lo tanto los niños con concentraciones de bilirrubina por encima de este nivel deben ser cuidadosa y frecuentemente monitorizados con respecto a las indicaciones de trasplante para optimizar el tratamiento pretrasplante.

Frühe Prognosefaktoren nach Kasai-Operation bei Kindern mit Gallengangatresie

Ziel der Studie: Evaluation von frühen Prognosefaktoren für eventfreies Überleben (keine Listung für Lebertransplantation, Lebertransplantation oder Tod) bei Kindern mit Gallengangatresie nach Hepatoportoenterostomie (Kasai-Operation). Patienten und Methoden: Innerhalb von 20 Jahren (1978 bis 1998) wurden 67 Kinder mit Gallengangatresie mit einer Kasai-Hepatoportoenterostomie behandelt. Das mediane Alter zum Zeitpunkt der Operation war 51 Tage (Bereich 19 - 180). Vierundzwanzig dieser Kinder mit vollständigem Datensatz und einer Mindestbeobachtungsdauer von einem Jahr wurden retrospektiv mittels Cox-Regressionsanalyse evaluiert. Als Response-Variable galt das eventfreie Überleben nach einer medianen Beobachtungszeit von 4,9 Jahren (Bereich 1,11 - 10,37). Covariablen waren Alaninaminotransferase (ALAT), Cholinesterase, Bilirubin, Alter bei Kasai-Operation und Tracer-Aufnahme und ‐Exkretion bei einer hepatobiliären Sequenzszintigraphie (HBSS). Alle Variablen wurden sechs Wochen nach der Operation untersucht. Mittels einer Receiver Operating Analyse (ROC) wurde ein Schwellenwert für Bilirubin definiert. Ergebnisse: Die Tracer‐Exkretion in der HBSS hatte den höchsten prognostischen Aussagewert zur Vorhersage eines eventfreien Überlebens nach Kasai-Operation (log rank 18,68, p < 0,0001), gefolgt vom Bilirubin und ALAT als weitere signifikante Parameter im ersten univariaten Schritt des Cox-Regression-Models. Im folgenden multivariaten Schritt wurde die prognostische Aussagekraft nur noch vom Bilirubin verbessert (log rank 24,6, p < 0,0001). Mittels Roc‐Analyse konnte ein Cut-off-Wert für Bilirubin von 57 µmol/l mit einer Sensitivität of 80 % und Spezifität von 78,6 % definiert werden. Die Fünf-Jahres-Überlebensrate ohne Event war 100 % in der Gruppe mit einer guten Tracer-Exkretion und einem Bilirubin von ≤ 57 µmol/l. Bei den anderen Kindern lag sie nur bei 27 % (log rank test, p < 0,0001). Schlussfolgerung: Frühe Erfolgsprädiktoren der Kasai-Operation bei Kindern mit Gallengangatresie sind die Tracer-Exkretion dargestellt mit einer HBSS und ein Bilirubin von < 57 µmol/l sechs Wochen nach der Operation. Säuglinge mit einer höheren Bilirubin-Konzentration sollten engfristig sorgfältig überwacht werden, um frühzeitig die Indikation zur Transplantation zu stellen und das Prätransplantationsmanagement zu verbessern.
