Zusammenfassung
Die chronisch thromboembolische pulmonale Hypertonie (CTEPH) ist eine der häufigsten Formen der pulmonalen Hypertonie. Allerdings werden bei weitem nicht alle Fälle diagnostiziert und therapiert. Therapie der Wahl ist die pulmonale Thrombendarterektomie, aber nur etwa die Hälfte der Patienten ist operabel. Rund 15 % der operierten Patienten sprechen auf die Therapie nicht an, meist aufgrund residueller PHT bei fortgeschrittenem Gefäßumbau. Die medikamentöse Therapie ist bei inoperablen Formen und postoperativen Rezidiven erfolgversprechend.
Summary
Chronic thromboembolic pulmonary hypertension is one of the most common forms of pulmonary hypertension (PHT), but frequently remains undiagnosed and untreated. Pulmonary thrombendarterectomy is the treatment of choice, but only about 50 % of such patients are operable. About 15 % of patients are not improved by surgery, usually because of residual PHT with advanced vascular changes. Drug treatment has favorable effects in inoperable forms and in postoperative recurrence of PHT.
