Zusammenfassung

Etwa 8 - 10 von 1000 Lebendgeborenen leiden an einer angeborenen Fehlbildung des Herzens oder der großen Gefäße. Von den hämodynamisch relevanten Herzfehlern gelangten noch vor 60 Jahren lediglich 10-15 % „spontan” ins Erwachsenenalter. Die primär hohe Letalität angeborener Herzfehler ließ sich in den letzten Jahrzehnten auf weniger als 20 % senken. Somit erreichen derzeit in Deutschland von den etwa 6500 Kindern, die pro Jahr mit einem angeborenen Herzfehler geboren wurden, etwa 5500 das Erwachsenenalter. Die Gesamtzahl der Erwachsenen, die mit angeborenen Herzfehlern das Erwachsenenalter schon erreicht hat, wird auf mehr als 150 000 geschätzt.

Die hohe Überlebensrate ist auf die medizinischen Fortschritte der letzten Jahrzehnte, insbesondere die verfeinerte Operationstechnik sowie die moderne pädiatrisch- und internistisch-kardiologische Diagnostik und Therapie zurückzuführen.

Fast alle Patienten mit angeborenen Herzfehlern bedürfen während ihres gesamten Lebens wegen anatomischer oder elektrophysiologischer Rest- und Folgezustände spezieller kardiologischer Betreuung.

Wenngleich unstrittig ist, dass die meisten Erwachsenen mit angeborenen Herzfehlern einer speziellen medizinischen Betreuung bedürfen, so ist die praktische Umsetzung immer noch ein Problem. Eine optimale Betreuung dieser recht komplexen Herzerkrankungen ist vielfach nur durch Kooperation zwischen speziell ausgebildeten „kongenitalen” Kardiologen, Herzchirurgen und Fachärzten anderer Fachbereiche im Rahmen spezieller Arbeitsgruppen möglich. Von entscheidender Bedeutung ist aber auch die Einbeziehung und Weiterbildung von Hausärzten, Allgemeinmedizinern und Internisten, die in der Basisversorgung dieser Patienten eine entscheidende Rolle spielen.

Summary

The incidence of congenital heart disease is 8 to 10 per 1000 newborn. Due to the advances of heart surgery, medical treatment and interventional cardiology the primarily high mortality rate of relevant congenital heart disease has declined from 80 % to 20 %. Nowadays more than 80 % of children with relevant congenital heart disease reach adulthood. Currently approx. 150 000 adults with significant congenital heart anomalies live in Germany presently. Their number is rising continuously.

With the exception of an patent Ductus Botalli and a small atrial septal defect, which could be operated in early childhood curatively, all other congenital heart defects need regular control, since even after successful primary interventional or operative treatment significant residuae or sequelae have to be expected.

Optimal care of these patients begins in the childhood and must be continued down to the high adulthood. Almost all of them need a careful follow-up in order to recognize residuae or sequelae and to treat them in time.

The complexity of many congenital heart defects makes a multidisciplinary approach necessary. A specialized, interdisciplinary cooperation between cardiologists, pediatric cardiologists, cardiac surgeons and other specialists is mandatory.
