Zusammenfassung

Bei Ressourcenengpässen sind Wartelisten in der gesamten klinischen Medizin verbreitet. Aufgrund der demographischen Entwicklung wird die Bedeutung von Wartelisten in der Medizin generell zunehmen. Die Arbeit diskutiert exemplarisch das Problem für die Radioonkologie und präsentiert den ersten deutschen Beitrag in einer Debatte, die bisher durch englischsprachige Literatur bestimmt wird. Ziel ist eine rationale Problemanalyse und Lösungsvorschlag unter Berücksichtigung medizinethischer Prinzipien. Die vorliegende Analyse basiert auf der aktuellen Literatur zum Thema und auf der eigenen klinischen Erfahrung im Umgang mit Wartelisten.

Wenn Wartelisten zum Einsatz kommen, können alle zentralen ethischen Prinzipien verletzt werden. Eine oft beobachtete Folge bei Mangel an strahlentherapeutischen Ressourcen sind Rationierungen im Bereich der palliativen Radiotherapie. Da dies die distributive Gerechtigkeit verletzt, muss dieses Prinzip bei der Problemlösung besonders beachtet werden. Ein egalitäres Gerechtigkeitskonzept und die Evidenz basierte Medizin erlauben eine ethisch basierte Lösung dieses klinischen Problems. Des weiteren ist eine Leitlinie zu formulieren, die Empfehlungen zum Gebrauch von Wartelisten gibt.
