Im vorliegenden Heft beschreiben Brueck et al. ihre umfangreiche Erfahrung über die perkutane Anlage zwei- bzw. einlumiger Silikonkatheter als chronischer Gefäßzugang für die Hämodialyse (Seite 2529). Bei insgesamt 203 Patienten mit meist chronischem Nierenversagen wurde eine primäre Erfolgsrate von 100 % und eine periprozedurale Komplikationsrate von nur 5 % gesehen, was die hohe technische Kompetenz der Gruppe belegt. Infektionsbedingte Katheter-Explantation, die wichtigste Komplikation des Verfahrens, war nur einmal pro jeweils 73 Patientenmonate erforderlich. Die Autoren kommen zu dem Schluss, dass subkutan implantierte Dialysekatheter bei älteren Patienten mit limitierter Lebenserwartung einen sinnvollen alternativen Gefäßzugang zur Nierenersatztherapie darstellen. Bedeutet dies, dass die Autoren „das Ei des Columbus” zur Lösung des Gefäßzugangproblems gefunden haben?
