Wann kann nach einer Hirnblutung wieder mit einer Phenprocoumon-Therapie begonnen werden?

Frage: Bei einer 80-jährigen Patientin wurde wegen eines Mitralklappenersatzes und absoluter Arrhythmie bei Vorhofflimmern eine Phenprocoumon-Therapie durchgeführt. Vor 4 Wochen erlitt die Patientin eine Hirnblutung mit Hemiplegie rechts und Dysphagie, z.Zt. intravenöse Ernährung über ZVK. - Kann Phenprocoumon wieder gegeben werden, und falls ja, welcher INR-Wert ist anzustreben, oder ist Antikoagulation mit niedermolekularem Heparin ausreichend?
