Spindelzellige und/oder epitheloidzellige mesenchymale Tumoren des Gastrointestinums mit immunhistochemischem Nachweis einer CD117 (c-kit)-Expression werden als GIST klassifiziert [6] [9]. Zusätzlich zur definierenden CD117-Positivität exprimieren GIST in 60 - 70 % der Fälle auch das hämatopoetische Vorläuferzellantigen CD34.

Wesentlicher pathogenetischer Mechanismus für die GIST-Entstehung ist die kontinuierliche, ligandenunabhängige Aktivierung der Tyrosinkinase auf dem Boden einer aktivierenden Mutation im Rezeptor. Die häufigste Mutation findet sich im Exon 11, seltener in den Exonen 9, 13 oder 17 [6]. Eine Entwicklung von GIST aus mesenchymalen Stammzellen mit Differenzierungspotenzial in Cajal-Zellen ist anzunehmen [12].

Vor 1998 wurden GIST aufgrund ihrer Morphologie und ihres Immunphänotyps zumeist als leiomyogene oder auch neurogene Neoplasien interpretiert und entsprechend als Leiomyome, Leiomyoblastome, Leiomyosarkome, Schwannome etc. klassifiziert.

kurzgefasst: Als GIST werden spindelzellige und/oder epitheloidzellige mesenchymale Tumoren des Gastrointestaltraktes mit immunhistochemischem Nachweis einer CD117 (c-kit)-Expression bezeichnet. Genetische Veränderungen führen über eine ligandenunabhängige Aktivierung der Tyrosinkinase zur Entstehung von GIST.
