Darmtuberkulose: Eine klinisch-diagnostische Herausforderung

Die Darmtuberkulose ist eine seltene Erkrankung in den Industrieländern, stellt aber eine wichtige Differenzialdiagnose zu chronisch entzündlichen Darmerkrankungen (CED), insbesondere zum Morbus Crohn, dar. In Deutschland sind vor allem zwei Gruppen besonders prädisponiert, eine primär extrapulmonale einschließlich einer intestinalen Manifestation der Tuberkulose (TBC) zu entwickeln: Dies sind zum einen die ständig wachsende Zahl der Immunsupprimierten und zum anderen die Gruppe der Immigranten besonders aus Osteuropa und Südostasien. Die Diagnosestellung der Darm-TBC ist weiterhin schwierig und gelingt häufig erst an spezialisierten Zentren durch das enge Zusammenspiel von Klinikern, Pathologen, Radiologen und Mikrobiologen. Da die Diagnosestellung dem Patienten in aller Regel nach einer konsequenten Therapie zu einer kompletten Ausheilung verhilft und im anderen Fall einer Fehldiagnose eventuell in einen fatalen Verlauf der Erkrankung münden kann, soll an dieser Stelle diese seltene Erkrankung nochmals in das Bewusstsein des Klinikers gebracht werden.
