Diagnostik und operative Therapie beim Insulinom - Erfahrungen bei 40 Patienten

Hintergrund und Fragestellung: Insulinome sind überwiegend benigne solitäre und funktionelle neuroendokrine Pankreastumoren. Die Symptome sind vielfältig, die präoperative Lokalisationsdiagnostik häufig wenig sensitiv und die Heilungsrate durch operative Tumorentfernung sehr gut. Ziel der Studie war die Analyse, Evaluierung des eigenen Krankengutes bezüglich der präoperativen Lokalisationsdiagnostik und der Operationsergebnisse.

Methoden: Die Daten aller Patienten, die an der Klinik für Visceral-, Thorax- und Gefäßchirurgie der Philipps-Universität Marburg zwischen 1987 und 2003 wegen eines Insulinoms behandelt wurden, wurden im Rahmen einer prospektiven Beobachtungsstudie erfasst und retrospektiv ausgewertet. Die Verdachtsdiagnose wurde bei allen Patienten durch einen Hungerversuch bestätigt.

Ergebnisse: Im Beobachtungszeitraum wurden 40 Patienten (22 Frauen, 18 Männer) mit einem mittleren Alter von 52 Jahren (Spannbreite 12-87 Jahre) wegen eines Insulinoms operiert. In der präoperativen Lokalisationsdiagnostik betrugen die Sensitivitäten von Endosonographie (EUS), Sonographie (US), Computertomographie (CT), Kernspintomographie (MRT) und Somatostatin-Rezeptor-Szintigraphie (SRS) 65 %, 33 %, 33 %, 15 % und 0 %. Im Gegensatz dazu, konnten alle Insulinome intraoperativ durch Pankreasfreilegung und intraoperative Ultrasonographie (IOUS) identifiziert werden. 5 Patienten wurden wegen eines malignen Insulinoms operiert. 38 Patienten konnten durch die operative Entfernung des Tumors dauerhaft geheilt werden.

Schlussfolgerung: Nach biochemischer Sicherung des Insulinoms durch einen adäquaten Hungerversuch und Ausschluss einer diffusen abdominellen Metastasierung durch transabdominellen Ultraschall, sollte der Patient ohne weitere präoperative Lokalisationsdiagnostik der Operation mit IOUS und bidigitaler Pankreaspalpation zugeführt werden. Eine Ausnahme besteht bei einer laparoskopisch geplanten Operation, dort sollte präoperativ zusätzlich eine Endosonographie durchgeführt werden.

Background and objective: Most insulinomas are solitary, benign and functional neuroendocrine pancreatic tumors which give rise to manifold symptoms. Their preoperative localization is often unclear, but the cure rate after their excision is very high. It was the aim of this study to analyse and evaluate our group of patients with regard to preoperative tumor localization and overall surgical results.

Methods: Data were collected as part of prospective observations and retrospective evaluation of all patients treated for insulinoma between 1987 and 2003 at the department of visceral- thoracic- and vascular surgery at the Philipps University of Marburg. In all of them the diagnosis had been confirmed by a fasting test.

Results: 40 patients with an insulinoma (22 females, 18 males; average age 52 years [range 12-87 years]) had been operated. The sensitivity of preoperative localization was 65% for endoscopic ultrasound, 33% for ultrasound, 33% for computed tomography, 15% for magnetic resonance imaging and 0% for somatostatin-receptor scintigraphy. But all insulinomas were identified intraoperatively by pancreas dissection and ultrasound (IOUS). 38 patients were completely cured by excision of the tumor.

Conclusion: After positive biochemical and fasting tests and exclusion of diffuse abdominal metastases by transabdominal ultrasound, all patients should without further preoperative investigations undergo surgical excision of the insulinoma after bidigital palpation of the pancreas and IOUS. But if laparoscopic excision is planned, endoscopic ultrasound should be undertaken preoperatively.
