An Unusual Presentation of Patent Urachus: Report of a Case

Abstract

Congenital umbilical disorders are frequently seen in the pediatric population and are mostly classified as retained umbilical cord structures or failure of umbilical ring closure. Vitelline duct anomalies, urachal anomalies and umbilical herniations comprise the main group of these congenital problems. Here we report a new presentation of vesicoumbilical patency in a 20-day-old male baby. A brief review of the literature concerning the classification of patent urachus to explain the possible mechanism of the anomaly is given.

Les anomalies congénitales de l'ombilic sont fréquentes dans la population pédiatrique, le plus souvent en rapport avec la persistance du canal omphalo-mésentérique ou un défaut de fermeture de l'anneau ombilical. Les anomalies du canal vitellin, les anomalies de l'ouraque et les hernies ombilicales représentent le principal groupe de ces problèmes congénitaux. Ici nous désirons rapporter un nouveau cas de pathologie ombilical chez un jeune garçon de 20 jours. Une brève revue de la littérature concernant la classification des ouraques sera réalisée pour expliquer les possibles mécanismes de cette anomalie.

Resumen

Se ven frecuentemente malformaciones congénitas del ombligo en niños que se suelen clasificar como estructuras del cordón umbilical persistentes o fallos del cierre del anillo umbilical. Las más importantes son las anomalías del conducto vitelino, del uraco, o las hernias umbilicales. Presentamos aquí un nuevo caso de comunicación vesico-umbilical patente en un niño varón de 20 días. Realizamos una breve revisión de literatura sobre la clasificación del uraco patente para explicar los mecanismos de la anomalía.

Zusammenfassung

Angeborene Fehlbildungen des Nabelbereiches werden bei Kindern sehr oft beobachtet und meist Reststrukturen der Nabelschnur zugeordnet, die im Rahmen eines fehlenden Verschlusses des Nabelringes persistieren. Anomalien des Ductus Vitelinus, Urachusfehlbildungen und Nabelhernien sind die Hauptgruppen dieser angeborenen Fehlbildungen. In der vorliegenden Arbeit wird ein besonderer Fall einer vesiko-umbilikalen Öffnung bei einem 20 Tage alten männlichen Knaben vorgestellt. Es wird eine Literaturübersicht gegeben und eine Klassifikation des offenen Urachus dargestellt sowie der pathogene Mechanismus versucht zu eruieren.
