Das Primärziel in der medikamentösen Therapie von Schilddrüsenknoten muss die Prävention sein, da Schilddrüsenknoten grundsätzlich das Risiko maligner Entartung bergen [1] [2] [18]. Unter Therapie mit Schilddrüsenhormonen verschwinden Knoten im Vergleich zu unbehandelten Kontrollgruppen vermehrt [37] bzw. unter Therapie treten weniger häufig neue Knoten auf [29].
Die zunehmende Größe von Schilddrüsenknoten kann, obwohl kontrovers diskutiert, zunehmend das Risiko einer malignen Entartung beinhalten [5] [17]. Daher kann auch eine Größenreduktion als weiteres Therapieziel gelten.
Klinisch und/oder sonografisch gesicherte Knoten (> 1 cm) müssen vor einer medikamentösen Therapie mittels Feinnadel-Aspirations-Zytologie (FNAC) abgeklärt werden [4] [10] [12] [24] [34]. Die sonografisch gestützte FNAC kann die hohe Operationsrate suspekter, letztlich doch benigner Schilddrüsenknoten und damit die Thyreoidektomie bei sonografisch falsch-positiven Kriterien für das Vorliegen von Malignität senken [4] [27] [34]. Von 134 Patienten mit ursprünglich benigner FNAC wies nach 11 Jahren Verlaufsbeobachtung ein Patient ein zytologisch diagnostiziertes und anschließend histologisch gesichertes Schilddrüsenkarzinom auf [18].
