Cholestatische Lebererkrankungen: Diagnostik und Therapie der primär sklerosierenden Cholangitis

Der konkrete Fall

Anamnese und klinischer Befund Bei einem jetzt 45-jährigen Mann wurde vor 8 Jahren eine Colitis ulcerosa diagnostiziert. Die anfänglichen Beschwerden (Blutbeimengungen im Stuhl) klangen unter Therapie mit Mesalazin 3 g/Tag rasch ab. Derzeit nimmt er noch 1,5 g Mesalazin/Tag. Vor etwa 5 Jahren fiel bei einer Routinekontrolle folgende Laborkonstallation auf: GOT 25 U/l, GPT 28 U/l, alkalische Phosphatase 350 U/l, gGT 180 U/l. Bilirubin 0,9 mg/dl. p-ANCA positiv. Untersuchungen Bei der Koloskopie zeigte sich eine inaktive Colitis ulcerosa mit Befall des gesamten Colon, zahlreiche kleine Pseudopolypen zwischen Mitte des Colon transversum und Rectum. Keine höhergradigen Dysplasien. Die ERCP ergab mehrere mittelgradige Stenosen der intrahepatischen Gallengänge. Die extrahepatischen Gallenwege waren unauffällig. Diagnose, Therapie und Verlauf Es wurde eine PSC bei Colitis ulcerosa diagnostiziert und eine Therapie mit UDC 4 × 250 mg/Tag eingeleitet (bei weiterhin 3 × 500 mg Mesalazin/Tag). Die Werte besserten sich für die folgenden 4 Jahre. Aktuell stiegen Transaminasen und Cholestaseenzymen wieder an. Bei einer ERCP wurden jetzt zwei hochgradige Stenosen im Gallenwegssystem nachgewiesen: eine im rechten Ductus hepaticus knapp vor der Hepaticusgabel, eine zweite im linken Leberlappen peripher. Die Stenose rechts konnte nach Papillotomie erfolgreich dilatiert werden. Die linksseitige Stenose kann nicht erreicht werden. Nach dem Eingriff kam es zu einer Cholangitis, die mit Ceftriaxon erfolgreich behandelt wurde. Als Therapie erhält der Patient jetzt UDC 4 × 250 mg/Tag, Mesalazin 3 × 500 mg /Tag. Geplant sind eine erneute Koloskopie mit der Fragestellung kolorektales Karzinom oder höhergradige Dysplasien sowie eine CT und MRI des Abdomens mit der Fragestellung: cholangiozelluläres Karzinom.
