Welchen Beitrag kann die Endoskopie bei der Diagnostik und Therapie der akuten Pankreatitis leisten? Spontan wird dem Leser der eingeklemmte Stein bei der akuten biliären Pankreatitis als fraglose Indikation zum endoskopischen Eingriff einfallen. Bei genauer Betrachtung der verschiedenen Formen der akuten Pankreatitis ergeben sich aber sehr viel mehr endoskopische Indikationen - insbesondere, wenn auch der endoskopische Ultraschall (EUS) mit berücksichtigt wird.
[Tab. 1] zeigt die verschiedenen Konstellationen, die eine endoskopische Diagnostik oder Therapie erfordern. Die Endoskopie kommt zum Zuge in den ersten Stunden einer durch Gangobstruktion ausgelösten akuten Pankreatitis, dann nach einigen Wochen bei der Behandlung infizierter Nekrosen oder der Diagnostik und Therapie unklarer morphologischer Befunde. Auf traumatische Läsionen des Pankreasganges (»Fraktur« des Ganges nach Huftritt oder durch den Fahrradlenker) wird nicht weiter eingegangen, obwohl auch in diesen Fällen ein Pankreatitis-ähnliches Bild durch endoskopische Prothesenversorgung behandelt werden kann.
