Aspiration während einer Notfallkoloskopie unter Propofol

Zusammenfassung

Ein 45-jähriger Mann erlitt im Gefolge einer koloskopischen Dilatation einer Morbus-Crohn-Rezidivstenose eine anhaltende peranale Blutung, die erfolgreich mit Unterspritzung und Hämoclips zum Stillstand gebracht werden konnte. Da der Patient zwischen den beiden Untersuchungen gegessen hatte, kam es am Ende der Untersuchung zu einer massiven Aspiration. Im weiteren Verlauf entwickelte er eine beidseitige Aspirationspneumonie, die auch 18 Monate nach dem Ereignis die Berufsfähigkeit immer noch beeinträchtigt.

Abstract

A 45-years old patient developed peranal bleeding after dilatation of an anastomotic stenosis in Crohn’s disease. At the end of a successful emergency colonoscopy during propofol sedation massive aspiration occurred due to the fact that the patient had eaten twice between the initial and the second endoscopy. He developed extensive aspiration pneumonia bilaterally and is still suffering from impaired respiration.
