Zusammenfassung
Der Barrett-Ösophagus (BE) gilt als Folgeerscheinung einer chronischen gastroösophagealen Refluxerkrankung und geht mit dem erhöhten Risiko eines Adenokarzinoms einher. Die Diagnose besteht aus der endoskopischen Darstellung eines veränderten Epithels im distalen Ösophagus und dem histologischen Nachweis einer intestinalen Metaplasie vom spezialisierten Typ. Beim Nachweis eines BE muss eine leitliniengerechte Überwachung stattfinden, die Intervalle richten sich zur Zeit nach der Länge des BE und dem Grad der intraepithelialen Neoplasie (IEN). Zur besseren Detektion von IEN kommen verschiedene Techniken zum Einsatz. Durchgesetzt hat sich die hochauflösende Videoendoskopie. Beim Nachweis hochgradiger Dysplasien ist eine Therapie erforderlich; zur Verfügung stehen endoskopisch-ablative und -resektive Verfahren. Auch mukosale Karzinome können endoskopisch reseziert werden, ab einer Infiltrationstiefe bis in die Submukosa (T1b) ist eine Operation indiziert.
