Akute nicht-allergische Reaktionen bei Anwendung von Depot-Penicillin

Zusammenfassung

Es werden akute nicht-allergische Reaktionen beschrieben bei intramuskulärer Anwendung procainfreier und procainhaltiger Depot-Penicillin-Präparate, die Kristalle einer bestimmten Größenordnung (30—60 µ) enthalten. Dabei kommt es zu unangenehmen, vorwiegend zentralen Symptomen mit akustischen und optischen Sensationen, die jedoch sämtlich wieder abklingen, ohne jemals fatal zu enden. Sie treten etwa bei jeweils 300 bis 400 Injektionen dieser Präparate einmal auf. Offenbar handelt es sich dabei um Kristall-Embolien durch akzidentelles Eindringen von Spritzmaterial in die Blutbahn, wobei Injektionsdruck und Gewebsschädigungen infolge vorausgegangener Injektionen eine Rolle zu spielen scheinen. Wir nehmen an, daß bei einem solchen Zwischenfall die Größe der Kristalle die Schwere der Symptome bestimmt und daß die Dauer der Reaktion von der Löslichkeit der Kristalle im Blut abhängt. Nach Verkleinerung der Kristallform von Megacillin^® (Clemizol-Penicillin-G) unter 20 µ wurde bisher bei über 1200 Injektionen nur einmal eine solche Reaktion gesehen, die zudem in erheblich abgeschwächter Form verlief.

Summary

Acute non-allergic reactions after intramuscular administration of procaine-free and procaine-containing depot penicillin preparations made up of crystals 30 to 60 µ in size are described. Unpleasant symptoms, especially of central character, occur in association with acoustic and optic sensations but always disappear and never end fatally. Such reactions occur about once in 300 to 400 injections. They are probably due to emboli by the crystals as the result of their accidental injection into the blood-stream; pressure of the injection and tissue damage from previous injections apparently play a role in facilitating this. The size of the crystals probably determines the severity of the symptoms; the duration of the reaction depends on the solubility of the crystals in blood. After the size of the crystals in penicillin-G had been reduced to below 20 µ, only one reaction, a rather mild one, was observed in 1200 injections.

Reacciones agudas no alérgicas en el empleo de penicilina depósito

Se describen reacciones agudas no alérgicas en el empleo intramuscular de preparados de penicilina depósito con y sin procaína, cuyos cristales abarcan una determinada gran escala (30—60 µ). Además sobrevienen síntomas desagradables, predominantemente centrales, con sensaciones acústicas y ópticas, las cuales, sin embargo, vuelven a desaparecer sin que jamás terminen fatalmente. Estas aparecen aproximadamente una vez de cada 300 a 400 inyecciones de este preparado. Se trata, evidentemente, de embolias de cristales debidas a un enclavamiento accidental de la substancia inyectada en el torrente sanguíneo, por lo que parece tener importancia la presión con que se inyecta y la lesión de los tejidos causada por inyecciones anteriores. Nosotros consideramos que el tamaño de los cristales determina la gravedad de la sintomatología en tales accidentes y que la duración de la reacción depende de la solubilidad de los cristales en la sangre. Después del empequeñecimiento de la forma de los cristales de megacilina (clemizol-penicilina G) por debajo de 20 µ sólo se vió una vez dicha reacción, la cual evolucionó, además, de una forma considerablemente atenuada.
