Therapie der entzündlichen Thrombophlebitis

Zusammenfassung

Tiefe und oberflächliche Thrombosen sind nur äußerst selten infektiös bedingt. Daher hat die Anwendung antibiotischer Mittel nur selten einen Sinn. Phenylbutazon (Butazolidin^®) wirkt zur Behandlung tiefer Thrombosen genau so gut wie bei oberflächlichen, wenn gleichzeitig eine gute Kompression angelegt wird. — Unter einem guten Kompressionsverband heilt eine Thrombose sehr rasch aus, zusammen mit Phenylbutazon meist bedeutend rascher als mit Antikoagulantien. Für die Behandlung einer oberflächlichen Thrombose braucht man niemals Antikoagulantien, sie reagiert schon innert einigen Tagen, ohne Arbeitsunterbrechung und ohne Bettruhe, auf eine entsprechende Therapie, ohne immer wieder zu rezidivieren. Sie ist, so behandelt, niemals eine Crux medicorum. — Ein guter Kompressionsverband ist vor allem auch für die Prophylaxe der Thrombose außerordentlich wirksam. Eine solche Vorbehandlung vor Operationen und vor Geburten, während der ganzen oder wenigstens während der zweiten Hälfte der Schwangerschaft, verhütet die so oft vorhandenen Beinödeme, die meistens gar nicht festgestellt werden. Das ergibt, zusammen mit dem Sofortaufstehen eine Thromboseprophylaxe, die eine zehn- bis dreißigfache Erniedrigung der Thrombosehäufigkeit bedingt. — Auch eine „Phlebitis” migrans heilt unter Kompression und Phenylbutazon, bei vollkommener Nicotinabstinenz aus. — Lymphödeme sind nie die, Folge einer Thrombose, sondern einer Lymphgefäßverlegung, meistens durch ein früher überstandenes Erysipel oder einen Tumor bedingt. — Zinkleimverbände ergeben nie eine genügend wirksame Kompressiqn. Ein Kompressionsverband sollte immer elastisch sein. Er muß täglich neu angelegt werden. Nur bei akuter oberflächlicher oder tiefer Thrombose wird ein Porelast- oder Poropan-Klebeverband angelegt, der einige Tage belassen wird. Darüber kommt aber jeden Tag frisch der Verband mit elastischen Varidress-Binden. Sogar bei arteriellen Erkrankungen hat eine gut angemessene Kompression beste Wirkung.

Deep and superficial thromboses are only rarely caused by infection; antibiotics are thus of little value. Phenylbutazone (Butazolidine^®) has proved of great value in both deep and superficial thromboses, in combination with compression bandages. This form of treatment is generally more effective than the use of anticoagulants. Superficial thromboses respond within a few days, without bed-rest or diminished activity, and do not recur again and again when so treated. Compression bandage is also of great prophylactic value, especially if applied before operation or delivery (during entire pregnancy or at least last half of it). Phlebitis migrans similarly heals quickly when treated with compression bandages and phenylbutazone, as long as there is strict avoidance of nicotine. — Lymphoedema is never the result of thrombosis but of changes in the lymphatic drainage as a consequence of erysipelas or tumour. Compression bandages should always be of the elastic type and should be changed daily (except in acute thrombosis when it is left on for a few days). Such compression bandages have proved of value also in the treatment of peripheral arterial disease.

Tratamiento de la tromboflebitis inflamatoria

En el tratamiento de las enfermedades de las venas es importante tener ideas claras acerca de la etiología. Una flebitis propiamente dicha, como se presenta en el 99% de todos los casos, nunca es de naturaleza infecciosa y consiguientemente los antibióticos no están indicados. La trombosis superficial se diferencia de la profunda sólo en lo que respecta a la localización y complicaciones, pero desde el punto de vista anatomopatológico tienen ambas idéntica causa. Es de gran importancia para prevenir y curar la trombosis, sea superficial o profunda, el tratamiento mediante compresión. De esta manera se redujo la frecuente trombosis que amenaza seriamente a las embarazadas de un 31,5% a un 0,9%. La fenilbutazona (Butazolidina^®) es, al contrario de lo que opina J. Jürgens, tan eficaz en una trombosis profunda como en una superficial, cuando se la administra convenientemente, es decir en inyección y siempre conjuntamente con la venda compresiva. El tratamiento mediante compresión de las trombosis profundas es también necesario y eficaz cuando el paciente está encamado.
