Die neurologischen Komplikationen bei maligner Lymphogranulomatose^*

Zusammenfassung

Von 403 Patienten mit histologisch gesicherter Lymphogranulomatose, die in den Jahren 1941 bis 1959 in der Medizinischen Universitätsklinik Freiburg behandelt wurden, hatten 47 Patienten durch die Lymphogranulomatose bedingte Komplikationen des Nervensystems. Die Symptomatologie, Diagnose und Differentialdiagnose bei Befall des Nervensystems durch die Lymphogranulomatose werden besprochen. Die Lokalisation der lymphogranulomatösen Herde, das Stadium des Krankheitsprozesses, die Verteilung der neurologischen Komplikationen nach Alter und Geschlecht, die Krankheitsdauer und Lebenserwartung sowie der Einfluß der Behandlung auf den Krankheitsverlauf werden in Beziehung zueinander gesetzt. Nach den vorliegenden Untersuchungen ist im Gegensatz zu anderen Autoren ein Befall des Nervensystems nicht nur bei der sarkomatösen Form des M. Hodgkin möglich. Der Anteil der neurologischen Komplikationen bei der Lymphogranulomatose wird mit 11,7% errechnet. Die in der Weltliteratur mitgeteilten Prozentsätze (27,7% und 36%) erscheinen zu hoch. Die Gründe hierfür werden diskutiert. — Besonders instruktive Krankheitsverläufe werden ausführlicher dargestellt.

Summary

Of 403 patients with histologically confirmed Hodkin's disease treated at the University Medical Clinic, Freiburg, between 1941 and 1959, 47 had neurological signs caused by the disease. The diagnostic and differential diagnostic features of these complications are described and discussed. The interrelationship between localization of the disease focus, the stage of the disease, the distribution of the neurological signs as to age and sex, the duration of the disease, the life-expectancy, and the influence of treatment on the course is analysed. Contrary to the findings of others, involvement of the nervous system was not restricted to the sarcomatous form of the disease in this series. The over-all incidence of neurological complications was 12%: the incidence of 28—36% reported by others is thought to be too high. The possible reasons for the discrepancy are discussed. Some particulary instructive case-reports are given.

Las complicaciones neurológicas en la linfogranulomatosis maligna (Conclusión)

De 403 enfermos con linfogranulomatosis maligna comprobada histológicamente que fueron tratados en la Clínica Médica Universitaria de Friburgo desde 1941 a 1959, 47 pacientes presentaban complicaciones de la linfogranulomatosis por parte del sistema nervioso. Se insiste sobre la sintomatología, diagnóstico directo y diagnóstico diferencial de la afectación del sistema nervioso en la linfogranulomatosis. Se estudiaron sucesivamente la localización de los focos linfogranulomatosos, el estadio del proceso morboso, la distribución de las complicaciones neurológicas según edad y sexo, la duración de la enfermedad y la probabilidad de supervivencia, así como la influencia del tratamiento sobre el curso de la enfermedad. Según las presentes investigaciones, la afectación del sistema nervioso resulta posible sólo, en contra de lo que piensan otros autores, en la forma sarcomatosa de la enfermedad de Hodgkin. Se calcula en un 11,7% el porcentaje de complicaciones neurológicas en la linfogranulomatosis. Las cifras de un 22,7% y un 36% que se encuentran al revisar la literatura parecen demasiado elevadas, a juicio de los autores. Se discuten los fundamentos que existen para opinar así. Se detallan los casos clínicos singularmente instructivos.
