Zusammenfassung

Spezifische klinische Symptome und Befunde, ähnlich denen bei Myasthenia gravis, wurden bei 20 Patienten beobachtet. Deutliche und reproduzierbare Unterschiede im Vergleich mit der typischen Myasthenia gravis machten es jedoch wahrscheinlich, daß unsere Patienten an einer anderen Erkrankung litten. Die klinischen und elektrophysiologischen Besonderheiten dieses neuen Syndromes wurden beschrieben. Das Vorhandensein eines malignen pulmonalen Tumors, hauptsächlich eines kleinzelligen Karzinoms, bei 14 der 20 Patienten war von besonderem Interesse. Die Pathogenese des Syndromes ist bisher ungeklärt. Dem Syndrom wurde der Name „Myasthenisches Syndrom, häufig von bronchiogenem, kleinzelligem Karzinom begleitet”, gegeben.

Summary

Specific signs and findings resembling those of myasthenia gravis were observed in 20 patients. But definite and reproducible differences from myasthenia gravis made it probable that the patients had a different disease, which—on the basis of clinical findings and electrophysiological tests—represent a new syndrome. A malignant intrathoracic small-cell carcinoma was present in 14 of the patients. The pathogenesis of the syndrome is as yet unknown. The condition has been given the descriptive name “myasthenic syndrome, frequently associated with small-cell bronchogenic carcinoma”.

Resumen

Un síndrome miasténico en estrecha relación con determinados tumores intratorácicos malignos

Se observaron en 20 enfermos síntomas clínicos específicos y hallazgos análogos a los de la miastenia grave. Sin embargo había algunas diferencias bien patentes, que podían ponerse de manifiesto repetidamente, que hacían plantearse la cuestión de que estos enfermos estuviesen afectos de otra enfermedad distinta. Se describen las singularidades clínicas y electrofisiológicas de este nuevo síndrome. Resultó de especial interés la existencia en 14 de los 20 enfermos de un tumor pulmonar maligno, sobre todo un carcinoma de células pequeñas. La patogenia de este síndrome no ha sido aclarada hasta el momento presente. Se ha propuesto para este síndrome la denominación de “síndrome miasténico acompañado a menudo de carcinoma bronquiogénico de células pequeñas”.
