Zusammenfassung

Der Schock in der Chirurgie ist zumeist durch Traumen ausgelöst. Blut und Plasmaverlust führen zum Volumenmangel, dem der Organismus durch die Zentralisation zu begegnen versucht. Eine möglichst frühzeitige Gabe kolloidaler Lösungen, am besten von Vollblut, kann weitere Spätfolgen vermeiden. Etwaige Blutungsquellen sind zunächst provisorisch und baldmöglichst endgültig zu schließen. Vor jedem Eingriff ist der Schock zu behandeln. Die Summation der Noxen durch das zusätzliche Operations- und Narkose-Trauma kann sonst einen erneuten Schockzustand auslösen. Die Behandlung hat sich nach dem Verlauf des Krankheitsbildes zu richten, wobei Blutdruck und Pulsfrequenz führende Hinweise geben. Eine Polypragmasie mit Gabe von Agonisten und Antagonisten ist abzulehnen. So einfach das typische Krankheitsbild des Schocks zunächst ist, so kompliziert kann sein weiterer Verlauf sich gestalten und bedarf deshalb einer den jeweiligen Gegegebenheiten angepaßten Therapie.

Summary

In surgery, shock is generally precipitated by trauma. The loss of blood and plasma leads to hypovolaemia to which the body responds by peripheral constriction. Early administration of colloidal solutions, optimally of whole blood, usually prevents later sequelae. In case of bleeding, the source obviously has to be dealt with either temporarily or, if possible, permanently. The treatment of shock should precede any other surgical procedure, because additional trauma due to the operation and/or anaesthesia may again cause shock. The frequent recording of blood pressure and heart rate give a useful hint about the course of the condition. Polypharmacy is rejected, and special problems of treatment are discussed.

El shock en la cirugía

El shock quirúrgico es provocado las más de las veces por traumatismos. La pérdida de sangre y plasma conduce a la deficiencia de volumen, procurando el organismo contrarrestarla por la centralización. La administración más pronta posible de soluciones coloidales, preferentemente de sangre completa, puede evitar otras secuelas tardías. Eventuales fuentes de hemorragia deben ser cerradas de momento provisionalmente y lo antes posible definitivamente. Antes de cada intervención debe tratarse el shock, ya que en caso contrario la sumación de factores nocivos por el traumatismo adicional operatorio y narcótico puede provocar un nuevo estado de shock. El tratamiento tiene que orientarse por el transcurso del cuadro patológico, dando indicaciones directoras la presión sanguínea y la frecuencia del pulso. Se debe rechazar una polipragmasia con administración de agonistas y antagonistas. Tan sencillo que es de momento el cuadro patológico típico del shock, tan complicado puede desarrollarse su curso ulterior, necesitando por lo tanto de una terapia adaptada a las situaciones concretas.
