Zusammenfassung

Auf Grund der hohen Toxizität des Dihydrostreptomycins und der Dihydrostreptomycin-Mischpräparate sowie der Gefahr ototoxischer Schäden auch bei Verwendung des Streptomycins und der Pantothensäure Verbindungen sollten die Streptomycine heute — abgesehen von der Tuberkulose-Behandlung — keine therapeutische Anwendung mehr finden. Das gilt ganz besonders auch für die Penicillin-Streptomycin-Kombinationen, bei denen sich das Risiko der toxischen Nebenwirkungen mit dem der antibakteriellen Unwirksamkeit addiert, da mehr als 90% der häufigsten gram-negativen Keime gegen Streptomycin resistent sind. Der Einsatz der Breitbandantibiotika, des Ampicillins bzw. der Cephalosporin-Antibiotika gewährleistet eine größere therapeutische Breite und Sicherheit. Penicillin-Streptomycin-Kombinationspräparate haben damit heute nur noch ein historisches Interesse, jedoch keinerlei sachliche Berechtigung mehr.

Summary

Because of the high toxicity of dihydrostreptomycin and dihydrostreptomycin-containing preparations, as well as the danger of damage to the ear — even when using streptomycin and pantothenic acid — streptomycin should nowadays only be used in treating of tuberculosis. This holds true in particular for penicillin-streptomycin combinations in which the risk of toxic side-effects is added to the development of bacterial resistance, which occurs in 90% of the common gramnegative micro-organisms. Broad-spectrum antibiotics, ampicillin or cephalosporin give the necessary therapeutic range with safety.

Sobre el tratamiento de infecciones bacterianas con preparados combinados de estreptomicina o dihidroestreptomicina y penicilina

Debido a la elevada toxicidad de la dihidroestreptomicina y de los preparados mixtos de dihidroestreptomicina así como al peligro de lesiones tóxicas del oído también en el empleo de estreptomicina y de las combinaciones de ácido pantoténico, no deberían encontrar las estreptomicinas, hoy en día, ninguna aplicación terapéutica, exceptuando el tratamiento antituberculoso, También esto es válido, principalmente, para las combinaciones de penicilina y estreptomicina, en las que se suma el riesgo de los efectos secundarios tóxicos con el de la ineficacia antibacteriana, ya que más del 90% de los gérmenes gram-negativos más frecuentes son resistentes a la estreptomicina. El empleo dé antibióticos de amplio espectro, de ampicilina o de los de cefalosporina garantiza un mayor espectro terapéutico y una mayor seguridad. Los preparados de combinaciones de penicilina y estreptomicina tienen, aún hoy en día, sólo un valor histórico, pero ninguna justificación objetiva.
