Operative Behandlung der Neurofibromatosis von Recklinghausen

Zusammenfassung

Für das Symptomenreiche Bild der Neurofibromatose von Recklinghausen gibt es kein festes Schema der Indikationsstellung zur Operation. In der Mehrzahl der Fälle herrscht aus pathomorphologischer Sicht das Neurofibrom vor. Das polymorph-histologische Bild erfordert eine umfassende Aufarbeitung der Tumoren, da die Abgrenzung adulter Neurinome (Schwannome) vom Neurosarkom schwerfallen kann. Aus der Erfahrung an zwölf Patienten, die im Laufe von 4 Jahren beobachtet werden konnten, ergibt sich der Schluß, daß eine konsequente Kontrolle von Kranken mit einer Neurofibromatose von Recklinghausen notwendig ist. Durch diese Kontrolle läßt sich der günstigste Zeitpunkt zur Operation bestimmen und damit das Operationsrisiko für den Kranken mindern.

Summary

Neurofibromatosis (von Recklinghausen's disease) has such manifold symptoms that there is no fixed set of indications for surgical treatment. In most cases the morphological picture is that of neurofibroma. The histological differentiation of the adult neurinoma (Schwannoma) from neurosarcoma may be difficult. Observations on 12 personal cases seen in the course of four years indicate that careful follow-up is essential in neurofibromatosis so that the optimal time for operation can be determined and the operative risk reduced.
