Zusammenfassung
Bei zwei Patienten im chronischen Hämodialyseprogramm kam es im Verlauf einer akuten Pankreatitis zum thrombotischen Verschluß der arteriovenösen Cimino-Fisteln jeweils an beiden Unterarmen. Als Ursache wurde eine systemische Hyperkoagulabilität (Auftreten von Fibrinmonomeren, Verkürzung der r- + k-Zeit im Thrombelastogramm, Verkürzung der partiellen Thromboplastinzeit) mit Fibrinolysehemmung (verlängerte Euglobulin-Lysis-Zeit) nachgewiesen. Die selektive Lokalisation der Thrombose an der Anastomosenstelle wird durch das Zusammentreffen von systemischer Hyperkoagulabilität und lokaler Plättchenadhäsivitätssteigerung erklärt.
Summary
An arteriovenous (Cimino) shunt in both right and left forearms clotted during an episode of acute pancreatitis in two patients on chronic haemodialysis. Systemic hypercoagulability (circulating fibrin monomers, shortening of r and k time in the thrombelastogram and shortening of the PTT-time) occurred in association with inhibition of fibrinolysis (prolongation of euglobulin lysis time). Localized thrombosis at the shunt is thought to be due to superimposition of systemic hypercoagulability on a local increase in platelet adhesiveness.
