Psychotische Symptome kommen bei einer Reihe psychischer Störungen vor. Psychotische Störungen verlaufen in Phasen. Unterschieden werden prämorbide und prodromale Phase, die Phase (Dauer) der unbehandelten Psychose (englisch: „Duration of Untreated Psychosis”; DUP), die Ersterkrankungsphase und die chronische bzw. residuale Phase. Die DUP ist deshalb von Bedeutung, weil sie mit biopsychosozialen Folgeschäden und Konsequenzen verbunden sein kann, die größtenteils eine schlechte Prognose im weiteren Verlauf der Erkrankung begünstigen. Psychotische Erstmanifestationen stellen hochkomplexe psychische Störungen dar. Dementsprechend hängen der kurz- bzw. langfristige Verlauf der Erkrankung und damit die Prognose maßgeblich von der Qualität der Erstbehandlung ab. Hier müsste das bestehende Versorgungssystem zugunsten von regional organisierten, spezialisierten Zentren in integrierter Zusammenarbeit mit niedergelassenen Nervenärzten umstrukturiert werden, die von Anfang eine langfristige, setting- und altersübergreifende, multimodaler Behandlung anbieten.
