Die Komplikationsrate nach Kryochirurgie an langen Röhrenknochen - eine tierexperimentelle Studie

Summary

In-vitro studies indicate that new miniature cryoprobes are suitable for cryoablation of bone tissue. Purpose of this experiment in 24 sheep was to examine the perioperative complications - in particular the danger of embolism - of cryoablation with miniature cryoprobes.

Cryoablation with 2 freeze-thaw-cycles each was carried out in the right tibia epiphysis and the left femur metaphysis. Pulmonary artery pressure (PAP) and central venous pressure (CVP) were measured. Throughout the intra- and perioperative phase heart rate and oxygen saturation by pulse oxymetry, blood gas and electrolytes were monitored regularly. Postoperative complications were examined.

A result, no significant increase of PAP, CVP or heart rate was measured. During the entire operation the results of the blood gas analyses were inconspicuous; pO2 and pCO2 were constant; for pH, standard bicarbonate and baseexcess an isolated insignificant change to slight acidosis was noted. The average decrease of HB was 0,5 g/dl. In one animal postoperative wound infection and wound necrosis was observed.

Occurrence of major perioperative complications of cryosurgical treatment of bone tissue, especially regarding clinically relevant pulmonary embolism, were not confirmed in experiments with sheep. Using new types of miniature cryoprobes, cryosurgery appears to be a safe alternative or addition to resectional procedures for treating abnormal bone tissue.
