Zusammenfassung

Es wird über das Ergebnis von 402 Goniotrepanationen berichtet. In 61% lag ein Glaucoma simplex vor, in 21% ein Glaucoma chronicum inflammatorium, in 11% ein Glaucoma acutum und 5% oder 2% waren sekundäre oder juvenile Glaukome. Die spontane Druckregulierung konnte bei einer Beobachtungszeit bis zu 7 Jahren in 77% erreicht werden, mit zusätzlicher lokaler Therapie und Bulbusmassage in 92,8%. Das Sehvermögen war in 76% unverändert oder besser, in 24% wurde eine Verschlechterung festgestellt. In keinem Fall kam es zu einem Gesichtsfeldausfall unmittelbar postoperativ. Zwischen der Druckregulierung und dem biomikroskopischen Aspekt des Sickerkissens ergab sich keine Korrelation. In 88,6% verlief die Operation komplikationsfrei, in 10,4% traten Frühkomplikationen auf und in 1% konnten Spätkomplikationen beobachtet werden. Beim Sekundärglaukom konnte eine spontane Druckregulierung in 45% der Fälle erreicht werden, von den 7 Fällen mit Hydrophthalmus gelang die Drucknormalisierung in 3 Fällen.

Summary

In a review of 402 goniotrephinations the preoperative diagnosis was open-angle glucoma in 61%, chronic congestive glaucoma in 21%, acute glaucoma in 11%, secondary glaucoma in 5%, and juvenile glaucoma in 2%. In 77% of the eyes followed for periods of up to seven years IOP returned to normal. Supplementary local therapy or digital eyeball massage raised the percentage of eyes with normal pressure values to 92,8% Visual acuity was maintained or improved in 76%. There was no case of visual field defect during the early postoperative period, and no correlation was found between the regulation of IOP and the condition of the filtering bleb. In 10,4% complications occurred during the early postoperativ period and in 1% late complications were seen. In 45% of the secondary glaucoma cases and in three out of seven cases of buphthalmus IOP was normalized.
