Zentralskotome bei fehlendem Marcus Gunn-Pupillenzeichen

Zusammenfassung

Trotz erheblicher unilateraler Visusreduktion kann das Marcus Gunn-Pupillenzeichen fehlen und der Swinging Flashlight Test negativ ausfallen. Dies trifft in der Regel für funktionelle Amblyopien, Medientrübungen und zentrale chorioretinale Läsionen zu. Bei Opticusläsionen ist hingegen ein normaler Pupillenbefund selten. Die möglichen Ursachen der Dissoziation von Visusreduktion und Störung des Pupillenlichtreflexes werden besprochen.

Summary

In spite of a massive unilateral reduction in visual acuity the Marcus Gunn pupillary sign may be absent and the Swinging Flashlight Test negative. As a rule this applies to patients with opaque media, functional amblyopia and central chorioretinal lesions. With optic nerve lesions, however, normal pupillary reactions are rare. The possible causes of dissociation of a decrease in central vision and disturbance of the pupillary light reflex are reviewed.
